摘要目的 了解目前广东省HIV/AIDS患者HIV-1基因型的流行状况.方法 用套式PCR对222例HIV/AIDS患者外周血单核细胞(PBMC)中HIV前病毒DNA膜蛋白env基因C2-V4区及其邻近区域片段进行扩增,并对片段进行序列测定,与国际HIV-1标准毒株序列进行比较,依据同源性确定基因型.结果 222例患者中,111例PCR扩增阳性并得到相应的序列,66例(59-46%)为CRF01-AE重组亚型,18例(16.22%)为BC型,18例(16.22%)为B亚型,9例(8.11%)为C亚型.不同性别、不同年龄组、不同感染来源的HIV-1感染均以CRF01-AE为主(均为50%以上).对抗逆转录病毒治疗12个月临床有效的57例患者,CD4+T细胞计数的变化在各基因型分组及各治疗方案分组间的差异无统计学意义(F=95.72、85.61,P＞0.05).结论 广东省HIV/AIDS患者中,目前流行的HIV-1毒株以CRF01-AE亚型为主,同时有BC(包括CRF07-BC、08-BC重组亚型)及B、C亚型存在,基因型分布与性别、年龄无明显相关性,基因型与抗逆转录病毒治疗方案的选择、疗效均无相关性.

abstractsObjective To explore the current status of HIV-1 genotype distribution in patients with HIV/AIDS in Guangdong province.Methods HIV-1 pro-viral DNA was extracted from PBMC,and env C2-V4 gene was amplified by PCR from 222 cases.The PCR product was subject to being sequenced.The acquired gene sequence was compared with that of standard strains to identify the genotype.Results HIV-1 genotype was successfully identified in 111 HIV/AIDS patients.66 patients(59.46%) had recombinant genotype CRF01-AE infection,18(16.22%) had recombinant BC type infection,and 18(16.22%)had subtype B,9(8.11%) had subtype C infection.CRF01-AE was predominant in all the patients with different gender,age and transmission route.All the 57 patients with AIDS who received highly antiretroviral therapy(HAART) for more than 12 months had good clinical response to the treatment.The differences of CD4+ cell counts among either the subtype groups or HAART regime groups were not obviously( F = 95.72,85.61,P＞0.05).Conclusions The current predominant circulated HIV-1 genotype is CRF01-AE in Guangdong province.Other genotypes include subtype BC(including CRF07-BC and CRF08-BC),subtype B and subtype C.The HIV-1 subtypes have no association with gender,age,the antiretroviral regimens or the effectiveness of treatment.
