摘要目的 比较经尿道钬激光前列腺剜除术与前列腺电切术及等离子电切术的疗效.方法 回顾性分析2016年2月至2017年11月在本院收治的良性前列腺增生症行手术治疗患者247例的临床资料,根据术式不同分为经尿道前列腺电切术(TURP组)86例,经尿道前列腺等离子电切术(PKRP组)104例,经尿道钬激光前列腺剜除术(HoLEP组)57例,对比术中及术后相关数据进行分析.结果 247例手术均成功,HoLEP组手术时间明显较TURP、PKRP组长,差异有统计学意义(P＜0.05);HoLEP组术后血红蛋白下降值明显低于TURP、PKRP组,差异有统计学意义(P＜0.05);术后血钠下降值HoLEP组明显低于TURP、PKRP组,差异有统计学意义(P＜0.05);导尿管留置时间HoLEP组明显短于TURP、PKRP组,差异有统计学意义(P＜0.05);HoLEP组术后住院时间较TURP、PKRP组明显缩短,差异有统计学意义(P＜0.05).三组术后并发症比较差异无统计学意义(P＞0.05).三组术后IPSS、QOL、Qmax较各自术前指标明显改善,差异有统计学意义(P＜0.05),三组之间比较差异无统计学意义(P＞0.05).结论 HoLEP与TURP、PKRP对BPH的短期疗效类似,HoLEP较TURP、PKRP出血更少,恢复更快.

abstractsObjective To compare and study the treatment efficiency between HoLEP,TURP and PKRP.Methods Retrospective analysis was made on the clinical data of 247 BPH patientsunderwent operation from February 2016 to November 2017.Eighty-six cases underwent TURP,104 cases underwent PKRP,and 57 cases underwent HoLEP.The operation time,bleeding volume during operation,hospitalization stay after operation,catheterization time were compared between the three groups.Results All 247 surgery were successfully performed.The operation time of HoLEP group were significantly longer than TURP and PKRP group(P ＜ 0.05).The HB declining after operation of HoLEP group were significantly fewer than TURP and PKRP group(P ＜ 0.05).The Na + declining of blood after operation of HoLEP group were significantly lower than TURP and PKRP group (P ＜ 0.05).The catheterization time of HoLEP group were significantly shorter than TURP and PKRP group (P ＜ 0.05).The hospitalization stay after operation of HoLEP group were significantly shorter than TURP and PKRP group(P ＜ 0.05).The incidence of complications had no significant difference between the three groups(P ＞0.05).The IPSS、QOL、Qmax after operation of three groups were significantly better than pre-operation(P ＜ 0.05) but had no significant difference between the three groups(P ＞ 0.05).Conclusions HoLEP has similar short-time curative effect with TURP and PKRP for BPH,but HoLEP has fewer bleeding and faster recovery than TURP and PKRP.
