摘要目的:探讨体重指数(BMI)与男性不育之间的相关性,从而为男性不育提供更多的治疗手段。方法:随机选取2017年7月至2019年7月南通市第一人民医院男科门诊就诊的不育患者,根据其BMI将患者分为四组:低体重组(BMI<18 kg/m 2)、正常体重组(18 kg/m 2≤BMI<24 kg/m 2)、超体重组(24 kg/m 2≤BMI<28 kg/m 2)和肥胖组(BMI≥28 kg/m 2);检测患者精液常规、性激素五项(睾酮、促卵泡生成素、泌乳素、促黄体生成素、雌二醇),并对结果进行统计处理。 结果:共纳入研究对象269例,其中低体重组13例、正常体重组96例、超体重组95例、肥胖组65例。四组患者的精液体积以及pH值比较,差异无统计学意义( P>0.05),而四组患者的精子密度、前向运动精子百分比、正常形态精子百分比比较,差异有统计学意义( P<0.05),其中所有患者的精液密度、前向运动精子百分比、正常形态精子百分比与BMI呈负相关( r=-0.365、-0.258、-0.468)。四组患者的睾酮和雌二醇比较,差异有统计学意义( P<0.05),睾酮与BMI呈负相关( r=-0.367);雌二醇与BMI呈正相关( r=0.352)。 结论:BMI可以影响男性精液质量以及性激素水平,引起不育。

abstractsObjective:To explore the correlation between body mass index(BMI) and male infertility, and to provide a theoretical basic treatment of male infertility.Methods:Patients from the male outpatient department of Nantong first people's hospital from July 2017 to July 2019 were randomly selected, the height and weight of the patients were measured, the body mass index was calculated. and all patients were divided into four groups according to their body BMI: low body weight group(BMI<18 kg/m 2), normal body recombination(18 kg/m 2≤BMI<24 kg/m 2), hyperrecombination(24 kg/m 2≤BMI<28 kg/m 2), and obesity group(BMI≥28 kg/m 2). The semen routine and five sex hormones(testosterone, follicle production, prolactin, luteinizing hormone, estradiol), and the results were statistically processed. Results:A total of 269 patients were included in the study, including 13 cases in low weight group, 96 cases in normal weight group, 95 cases in overweight group and 65 cases in obesity group.There was no statistical difference in semen volume and pH between the four groups( P>0.05), and there was a significant statistical difference in sperm density, forward motile sperm percentage, and normal morphology sperm percentage among the four groups( P<0.05). Semen density, percentage of forward motile sperm, and percentage of normal morphological sperm were negatively correlated with BMI( r=-0.365, -0.258, -0.468). The difference between testosterone and estradiol between the four groups was statistically significant( P<0.05). The negative correlation between testosterone and BMI index was -0.367, and the positive correlation between estradiol and BMI index was 0.352. Conclusions:BMI can affect semen quality and sex hormone levels in men, which can lead to infertility.
