摘要目的 为了研究磷酸二酯酶(PDE)7B在慢性淋巴细胞白血病(CLL)患者中的表达及临床意义.方法 应用实时定量逆转录PCR(qRT-PCR)技术检测87例未治疗CLL患者和20例正常人中PDE7B的表达水平,并分析其与CLL临床预后指标的关系.结果 87例CLL患者PDE7B表达水平中位数为5.4×10-4(0.2×10-4～1.5×10-2),明显高于正常人群0.5×10-4(0.18×10-4～1.7×10-4)(P＜0.001).PDE7B与年龄、Binet分期、免疫球蛋白重链基因(IgVH)突变、CD38表达及细胞遗传学明显相关,而与性别及ZAP-70表达无明显相关性.其中26例患者具有治疗指征并接受治疗,其PDE7B表达水平明显高于无治疗指征的患者(P=0.021).结论 PDE7B基因在MCL中表达明显高于正常人群,并与不利的细胞遗传学、Binet B和C期、IgVH基因无突变、CD38阳性关系密切,可能成为CLL新的预后指标,具有重要的临床意义.

abstractsObjective To investigate the expression level of cyclic nucleotide phosphodiesterase (PDE) 7B mRNA in chronic lymphocytic leukemia (CLL) and its prognostic value of PDE7B in CLL.Methods A total of 87 blood samples from untreated CLL patients were collected, real-time quantitative RT-PCR (qRT-PCR) was used to access PDE7B mRNA expression, and its association with prognostic index was analyzed by statistics software. Results The median values of PDE7B mRNA expression level in 87 CLL patients and normal control were 5.4 × 10-4 (0. 2 × 10-4 ～ 1.5 × 10-2) and 0.5 × 10-4 (0.18 × 10-4～ 1. 7 × 10-4), respectively (P ＜ 0. 001). The results showed that PDE7B mRNA expression was significantly associated with age, Binet stages, immunoglobulin heavy-chain variable region (IgVH)mutation status, CD38 expression and cytogenetic abnormalities, but not with gender and ZAP-70expression. PDE7B mRNA expression in 26 CLL patients who had treatment indications and received treatment was higher than untreated ones (P = 0. 021). Conclusion PDE7B mRNA expression was markedly higher in CLL patients compared with normal controls, which was significantly correlates with unfavorable cytogenetic characteristics, Binet stages B and C, unmutated IgVH and CD38 positive. It might be a novel prognostic indicator in CLL, which possess important clinical significance.
