万步运动对非透析慢性糖尿病肾病患者微炎症状态及肾功能的影响
Effects of Wanbu exercise on micro-inflammatory state and renal function in patients with nondialysis chronic diabetic nephropathy
摘要目的:探究万步运动对非透析慢性糖尿病肾病(diabetic nephropathy, DN)患者血糖水平及肾功能的影响。方法:以我院2015年1月至2019年10月收治的149例非透析慢性DN患者作为研究对象,依据治疗措施不同分为对照组和观察组,对照组74例给予基础治疗和营养方案,观察组75例在前者基础上增加万步运动方案。对比干预前、干预1个月末、3个月末两组患者微炎症状态、血糖水平和肾功能。结果:较干预前,两组患者干预1个月末及干预3个月末血浆C反应蛋白(CRP)、白细胞介素-6(IL-6)水平呈逐渐降低趋势,且观察组血浆CRP、IL-6水平低于对照组(均 P<0.05)。较干预前,两组患者干预1个月末及干预3个月末血清空腹血糖(FPG)、餐后2 h血糖(2-h PG)、糖化血红蛋白(HbAlc)水平呈逐渐降低趋势,且观察组血清FPG、2-h PG、HbAlc水平低于对照组(均 P<0.05)。较干预前,两组患者干预1个月末及干预3个月末血清尿素氮(BUN)、血肌酐(Scr)水平呈逐渐降低趋势,观察组血清BUN、Scr水平低于对照组(均 P<0.05)。 结论:万步运动可通过加快肌肉内血液循环、促使肾脏血流加快以促进非透析慢性DN患者肾脏毒素排出,改善肾功能,调节血糖水平,改善微炎症状态。

abstractsObjective:To explore the effects of Wanbu exercise on blood glucose level and renal function in patients with non-dialysis chronic diabetic nephropathy (DN).Methods:A retrospective analysis of 149 cases of non-dialysis patients with chronic DN was conducted. According to the different treatments, the subjects were divided into the control group and the observation group. In the control group, 74 patients were treated with foundation treatment and nutrition. In the observation group, 75 patients were treatecl with increased movement scheme. After 3 months, inflammatory state, kidney function and blood glucose level were compared before and after intervetion.Results:The plasma levels of CRP and IL-6 in the two groups were gradually decreased from January to the end of March compared with that before the intervention, and the plasma levels of CRP and IL-6 in the observation group were lower than those in the control group ( P<0.05). The serum levels of FPG, 2-h PG, and HbAlc in the observation group were lower than those in the control group from January to the end of March after intervention ( P<0.05). The levels of serum BUN and Scr in the observation group were lower than those in the control group ( P<0.05). Conclusions:Wanbu exercise can accelerate the blood circulation in the muscles and accelerate the blood flow in the kidney to promote the excretion of toxins in the non-dialysis chronic DN patients, thus improving the renal function, regulating the blood glucose level, improving the state of micro-inflammation, and improving the clinical efficacy.
