基于鼻窦CT影像的额窦中隔气房与额窦引流通道解剖关系的研究
Anatomical relationship between the frontal sinus septal cell and drainage pathway of the frontal sinus based on preoperative sinus CT
摘要目的:基于鼻窦CT及其三维重建影像,探讨额窦中隔气房与额窦引流通道的解剖关系。方法:横断面研究。纳入解放军总医院第六医学中心2023年12月—2024年5月经鼻窦CT检查,同侧存在额窦中隔气房及额窦的127例患者的CT影像资料,其中男78例、女49例,年龄18~81岁,中位年龄44岁。患者额窦中隔气房均为单一气房,左侧额窦中隔气房53例、右侧额窦中隔气房46例、双侧均有额窦中隔气房28例,共155侧。通过CT影像观察和分析额窦中隔气房与额窦引流通道的解剖关系。结果:额窦中隔气房和额窦引流通道位置均毗邻,根据冠状位CT显示额窦中隔气房的引流口位置,可以将额窦中隔气房与额窦的解剖关系分为4类:第1类,开口于钩突内侧(116侧,74.8%),额窦引流通道位于额窦中隔气房的外后方;第2类,开口于额窦前组气房(20侧,12.9%),额窦引流通道位于额窦中隔气房的后方;第3类,开口于筛泡上额气房(6侧,3.9%),额窦引流通道位于额窦中隔气房的前外侧;第4类,开口于额窦引流通道(13侧,8.4%),额窦引流通道位于额窦中隔气房的外侧。结论:不同类型额窦中隔气房与额窦引流通道的相对位置有所差异。由于额窦中隔气房的解剖位置较为固定,可作为内镜下定位额窦引流通道的标志。

abstractsObjective:This study aimed to explore the anatomical relationship between the frontal sinus septal cells and the frontal sinus drainage pathways on the basis of sinus CT scans and 3D reconstruction image.Methods:This work was a cross-sectional study that included CT imaging data of 127 patients who underwent sinus CT examinations at the Sixth Medical Center of the PLA General Hospital from December 2023 to May 2024. All patients had both frontal sinus septal cells and frontal sinuses on the same side. The cohort comprised 78 males and 49 females, aged 18-81 years, with a median age of 44 years. All frontal sinus septal cells were single, with 53 on the left side, 46 on the right side, and 28 with bilateral septal cells, totaling 155 sides. The anatomical relationship between the frontal sinus septal cells and drainage pathways of the frontal sinus was observed and analyzed using CT imaging.Results:Frontal sinus septal cells were closely adjacent to the drainage openings of the frontal sinus. On the basis of the location of the drainage openings of the frontal sinus septal cells shown in coronal CT images, the anatomical relationship between the frontal sinus septal cells and the frontal sinus can be classified into four categories. Type 1: opening on the medial side of the uncinate process (116 sides, 74.8%), with the drainage pathway located posterior-laterally to the frontal sinus septal cell; Type 2: opening in the anterior group of frontal recess cells (20 sides, 12.9%), with the drainage pathway located anterior-laterally to the frontal sinus septal cell; Type 3: opening into the supra bulla frontal cell (six sides, 3.9%), with the drainage pathway located anterior-laterally to the frontal sinus septal cell; and Type 4, opening into the frontal sinus ostium (13 sides, 8.4%), with the drainage pathway located laterally to the frontal sinus septal cell.Conclusion:The relative positions of different types of frontal sinus septal cells and frontal sinus drainage pathways vary. However, the anatomical location of the frontal sinus septal cells is relatively fixed, making it a potential landmark for locating the frontal sinus drainage pathways under endoscopy.
