摘要目的 探讨细胞DNA定量分析技术(DNA image cytometry,DNA-ICM)在乳腺癌诊断中的应用价值.方法 收集2014年8月至2017年10月因乳腺或腋窝肿块就诊于安徽医科大学第一附属医院普外科和肿瘤科患者173例,其中乳腺癌100例,良性疾病73例.局部穿刺标本分别行DNA-ICM和液基细胞学检查(liquid-based cytology,LBC)检查后,将2者结果进行对比.结果 DNA-ICM在乳腺癌诊断中的敏感度为82.0%、特异度为100.0%,LBC的敏感度为69.0%、特异度为97.3%,DNA-ICM的敏感度高于LBC,差异有统计学意义(P=0.033).所有乳腺组织中DNA-ICM检查的敏感度为83.3%,特异度为100.0%,LBC检查的敏感度为64.8%,特异度为97.5%,DNA-ICM检查的敏感度高于LBC(P=0.028).腋窝组织中DNAdCM检查的敏感度为80.4%,特异度为100%,LBC检查的敏感度为73.9%,特异度为97.0%,DNA-ICM的敏感度高于LBC检查,但2者差异无统计学意义(P=0.456).DNA-ICM联合LBC检查敏感度为93.5%,高于LBC检查(P=0.024),显示联合检查效果优于LBC.结论 DNA-ICM在乳腺癌诊断中具有较大价值,可与LBC联合应用于乳腺癌的诊断.

abstractsObjective To determine the value of DNA image cytometry (DNA-ICM) in diagnosis of breast cancer.Methods Totally 187 patients with breast or axillary mass admitted from Aug.2014 to Oct.2017 were included in the study.Fine-needle aspiration cytology (FNAC),DNA-ICM and liquid-based cytology (LBC) were employed.The results showed that 100 cases were breast cancer patients and 73 cases were benign patients.DNA-ICM and LBC were used to compare the sensitivity and specificity of the two methods.Results The sensitivity and specificity were 82.0% and 100% for DNA-ICM,and 69.0% and 97.3% for LBC.The sensitivity of DNA-ICM was significantly higher than that of LBC (P=0.033).In breast tissue,the sensitivity and specificity of DNA-ICM were 83.3% and 100%,and they were 64.8% and 97.5% for LBC.The difference of sensitivity was statistically significant (P=0.028).In axillary tissue,the sensitivity and specificity of DNA-ICM were 80.4% and 100%,and they were 73.9% and 97.0% for LBC.The difference was not statistically significant (P=0.456).The sensitivity of joint detection was 93.5%,significantly higher than that of LBC alone (P=0.024).The effect of joint detection was better than that of LBC.Conclusion DNA-ICM method is of great value in the diagnosis of breast cancer,which can be combined with LBC in diagnosis of breast cancer.
