摘要目的 探讨3R强化护理对提高阿尔茨海默病患者认知功能和生活质量的效果.方法 选择轻、中度阿尔茨海默病患者66例,随机分为实验组(n=33)和对照组(n=33).对照组接受内科常规护理外,实验组患者在此基础上实施往事记忆提取(Reminiscence)、记忆空间定位(Reality)和记忆再激发(Remotivation)的3R强化训练.采用工具性日常生活活动(ADL)、功能独立性评定(FIM)、成人韦克斯勒智力量表(WAIS-RC)和简易智力状态检查量表(MMSE)比较2组生活质量改善情况.结果 在6个月的观察期间,对照组MMSE和WAIS逐渐下降(P<0.05),实验组MMSE和WAIS无明显变化;对照组ADL和FIM无明显变化,实验组ADL和FIM评分比人组时有进步(P<0.05).结论 3R强化训练可改善AD患者认知功能,一定程度上促进了患者日常生活能力的恢复,为预防痴呆发展和提高患者生活质量的有效措施.

abstractsObjective To investigate the effect of 3R intensive nursing care for patients with Alzheimer Desease in cognitive function and quality of life.Methods Sixty-six of patients with AD in hospital were randomly divided into the experimental group and the control group.Patients in control group just accepted general medical care.Pailents in experimental group accepted general medical care and 3R intensive training,Reminiscence,Reality and Remotivation.All patients were evaluated respectively with IADL,FIM,WAIS-RC and MMSE about life quality improvement.Results Scores of MMSE and WMS of patients in the control group reduced after 6months.which did not change in the experimental group.Scores of ADL and FIM of patients in the control group did not change,but which improved in the experimental group(P<0.05).Conclusions Three R intensive training can improve cognitive function of patients with AD and can be helpful for their activities of daily living,which may be an effective measure for prevention of the aggravation of the patients'symptom and improving of patients'qualities of life.
