摘要目的 探讨基于快速康复理念下,早期肠内营养在机器人辅助腹腔镜前列腺根治性切除术患者围术期的应用效果.方法 采用便利抽样法于2016年1—12月选择解放军总医院泌尿外科收治的93例拟行机器人辅助腹腔镜前列腺根治性切除术的患者,应用随机数字表法将患者分为两组,其中对照组52例,采用常规护理模式下的营养支持方案进行护理;观察组38例,采用快速康复护理措施结合早期肠内营养的护理方案.观察比较两组患者胃肠功能恢复情况、营养支持情况及住院费用的差异.结果 观察组患者的首次排气时间和首次排便时间分别为(2.06±0.41)、(2.57±1.00)d,均早于对照组,差异有统计学意义(P<0.05);静脉营养时间及术后住院日均较对照组缩短,且住院费用少,差异均有统计学意义(P<0.05).结论 早期肠内营养能够有效促进机器人腹腔镜前列腺根治性切除患者的术后胃肠功能恢复,减轻患者术后不舒适,缩短术后住院时间,减少住院费用,值得临床推广.

abstractsObjective To discuss the effect of early enteral nutrition based on rapid rehabilitation nursing in perioperative nursing of patients underwent robot-assisted laparoscopic prostatectomy. Methods A total of 93 patients who would undergo robot-assisted laparoscopic prostatectomy from January to December 2016,were randomly divided into the control and experimental groups. Patients in the control group(n=52) received traditional nutrition support protocol,while patients in the experimental group(n=38)were given early enteral nutrition based on rapid rehabilitation nursing theory. Recovery of gastrointestinal function,status of nutrition support and economic index were compared between the two groups. Results The mean anal exhaust time and the mean defecation time in the experimental group were(2.06±0.41)d and(2.57±1.00)d respectively, which were significantly earlier than those in the control group(P<0.05). The mean time of parenteral nutrition and post-operative hospitalization days as well as the medical cost in the experimental group were significantly shorter than those in the control group(P<0.05). Conclusions Early enteral nutrition can effectively promote the recovery of gastrointestinal function of patients underwent robot-assisted laparoscopic prostatectomy,alleviate the post-operative discomfort,shorten the post-operative hospitalization days and reduce the medical cost.
