以护士为主导的多学科-信息化血糖管理模式在虚拟病区中的探索与应用

Exploration and application of multidisciplinary information-based blood glucose management dominated by nurses in virtual wards

摘要目的:构建以护士为主导的多学科-信息化血糖管理模式,提高非内分泌科联络护士的专科知识,实现全院血糖管理同质化。方法:借助互联网技术在医院建立多学科联动的血糖管理系统,于2018年8—12月,通过对58个病区共86名联络护士专业化培训,利用信息化手段构建虚拟病区,对非内分泌科糖尿病患者开展远程治疗。采用便利抽样法,从58个病区抽取10个病区,以2018年5月入住10个病区的212例糖尿病患者为对照组,采用院内一般会诊制度。2019年5月入住的238例糖尿病患者为试验组,进行多学科-信息化血糖管理。比较培训前后联络护士的糖尿病知识掌握情况以及两组患者的血糖管理质量和患者满意度。结果:培训后,联络护士糖尿病相关知识得分均提高,与培训前比较差异有统计学意义( P<0.05)。试验组患者出院时空腹血糖达标率97.48%、餐后2 h血糖达标率92.02%,住院期间糖化血红蛋白检测率83.61%,葡萄糖耐量检测率44.79%,患者满意度99.16%,均高于对照组,差异均有统计学意义( P<0.05)。 结论:打造虚拟病区,进行多学科-信息化血糖管理能够提高糖尿病患者血糖管理质量和满意度,具有一定的临床意义,值得推广。

abstractsObjective:To establish a multidisciplinary information-based blood glucose management dominated by nurses, improve the specialized knowledge of non-endocrinological liaison nurses, and achieve homogenization of blood glucose management in the whole hospital.Methods:A multidisciplinary blood glucose management system was established in the hospital with the help of internet technology. From August to December 2018, specialized training was provided to totally 86 liaison nurses from 58 wards. Virtual wards were constructed using information technology, and remote treatment was provided to diabetic patients. A total of 10 wards were selected out of 58 wards, and the 212 diabetic patients who were admitted to the 10 wards in May 2018 were included into the control group, and the general consultation system in the hospital was adopted for them. Totally 238 diabetic patients admitted in May 2019 were included into the experimental group, and were subjected to multidisciplinary information-based blood glucose management. The diabetes knowledge of the liaison nurses before and after the training as well as the blood glucose management quality and patient satisfaction of the two groups of patients were compared.Results:After the training, the diabetes-related knowledge scores of the liaison nurses increased, and the difference was statistically significant compared with that before the training ( P<0.05) . The fasting blood glucose compliance rate at discharge, 2 h postprandial blood glucose compliance rate, in-hospital glycosylated hemoglobin detection rate, glucose tolerance detection rate, and patient satisfaction of the experimental group were 97.48%, 92.02%, 83.61%, 44.79%, and 99.16% respectively, all higher than those of the control group, and the differences were statistically significant ( P<0.05) . Conclusions:Creating a virtual ward and implementing multidisciplinary informatized-based blood glucose management can improve the quality of blood glucose management and satisfaction of diabetic patients, which is of certain clinical significance and is worth promoting in clinical practice.
