摘要目的:探讨专科护士主导的患者自我报告结局监测在2型糖尿病患者居家管理中的应用效果。方法:采用便利抽样法选择2021年3—12月在新疆医科大学第一附属医院糖尿病专科门诊就诊的168例2型糖尿病患者，随机分为对照组和观察组，各84例。对照组采取常规居家护理管理，观察组在对照组的基础上实施以专科护士主导的患者自我报告结局监测干预。比较干预前后两组的血糖指标、糖尿病困扰程度[糖尿病症状困扰量表（DSC-R）和糖尿病问题量表（PAID）]以及干预后患者糖尿病治疗满意度。结果:3个月干预后，观察组的空腹血糖、餐后2 h血糖、糖化血红蛋白、DSC-R得分和PAID得分均低于对照组，且较干预前下降，差异有统计学意义（ P<0.05）；观察组的治疗满意度得分高于对照组，自我感觉高血糖得分和自我感觉低血糖得分均低于对照组，差异均有统计学意义（ P<0.05）。 结论:专科护士主导的患者自我报告结局监测可以有效改善糖尿病患者血糖水平，对缓解糖尿病困扰和提高治疗满意度方面具有一定优势。

abstractsObjective:To explore the effect of patient reported outcome measures led by specialist nurses in the home management of type 2 diabetes patients.Methods:From March to December 2021, 168 patients with type 2 diabetes were selected by convenience sampling from the Diabetes Clinic of the First Affiliated Hospital of Xinjiang Medical University. The patients were randomly divided into control group and observation group, with 84 cases each. The control group adopted routine home nursing management, and the observation group implemented patient reported outcome measures led by specialist nurses on the basis of the control group. The blood glucose index, diabetes distress degree [the Diabetes Symptom Checklist-Revise (DSC-R) and Problem Areas in Diabetes Scale (PAID) ] before and after intervention and diabetes treatment satisfaction of patients after intervention were compared between the two groups.Results:After 3-month intervention, the fasting blood glucose, 2-hour postprandial blood glucose, glycosylated hemoglobin, DSC-R score and PAID score of the observation group were lower than those of the control group, and the differences were statistically significant ( P<0.05) . The treatment satisfaction score of the observation group was higher than that of the control group, and the scores of self-perceived hyperglycemia and self-perceived hypoglycemia were lower than those of the control group, with statistical differences ( P<0.05) . Conclusions:Patient reported outcome measures led by specialist nurses can effectively improve the blood glucose of diabetes patients, and has certain advantages in alleviating the diabetes distress and increases treatment satisfaction.
