基于时空理念的情景模拟干预在缺血性脑卒中患者中的应用

Effect of situational simulation intervention based on space-time concept in patients with ischemic stroke

摘要目的:探讨基于时空理念的情景模拟干预在缺血性脑卒中患者中的应用效果。方法:2019年1月—2021年12月,采用便利抽样法选取长治市医学院附属和平医院收治的160例缺血性脑卒中患者,采用随机数字表法将其分为观察组和对照组,各80例。观察组实施基于时空理念的情景模拟干预,对照组给予常规干预。干预前、出院时及出院后6个月,比较两组简易智能精神状态检查表(MMSE)评分、美国国立卫生研究院卒中量表(NIHSS)评分、Barthel指数(BI)及心理韧性评定量表(CD-RISC)评分。结果:干预前、出院时及出院6个月,两组MMSE、BI及CD-RISC评分逐渐升高,且观察组高于对照组,差异有统计学意义( P<0.05);干预前、出院时及出院6个月,两组NIHSS评分逐渐降低,且观察组低于对照组,差异有统计学意义( P<0.05)。 结论:基于时空理念的情景模拟干预可以改善缺血性脑卒中患者的认知功能、日常生活活动能力、神经功能及心理韧性,在患者出院后仍有延续效果。

abstractsObjective:To explore the effect of situational simulation intervention based on space-time concept in patients with ischemic stroke.Methods:From January 2019 to December 2021, 160 patients with ischemic stroke admitted to Peace Hospital Affiliated to Changzhi Medical College were enrolled using the convenience sampling method, and divided into the observation group and the control group by the random number table method, with 80 cases each. The observation group conducted situation simulation intervention based on space-time concept, and the control group received routine intervention. Before intervention, at discharge and 6 months after discharge, the scores of Mini-Mental State Examination (MMSE) , National Institutes of Health Stroke Scale (NIHSS) , Barthel Index (BI) and Connor-Davidson Resilience Scale (CD-RISC) were compared between the two groups.Results:Before intervention, at discharge and 6 months after discharge, MMSE, BI and CD-RISC scores of the two groups gradually increased, and the scores of the observation group were higher than those of the control group, and the differences were statistically significant ( P<0.05) . Before intervention, at discharge and 6 months after discharge, NIHSS scores of the two groups gradually decreased, and the score of the observation group was lower than that of the control group, and the difference was statistically significant ( P<0.05) . Conclusions:Situational simulation intervention based on space-time concept can effectively improve patients' cognitive function, ability of daily living, neurological function and psychological toughness, and the effect persisted after discharge.
