ADC直方图在鉴别胶质母细胞瘤和脑单发转移瘤的应用价值

摘要目的 探讨ADC直方图在鉴别胶质母细胞瘤和脑单发转移瘤的应用价值.方法 回顾性分析2013年5月至2017年10月于郑州大学第一附属医院经病理证实的30例胶质母细胞瘤及28例脑单发转移瘤,用MaZda软件提取肿瘤的感兴趣区,得到9个直方图参数,对参数进行统计学分析,并绘制ROC曲线以评价直方图参数鉴别胶质母细胞瘤和脑单发转移瘤的价值.结果 直方图提取出的9个参数特征中,两组间平均值、偏度、第10百分位数、第50百分位数、第90百分位数的差异有统计学意义(P<0.05),其ROC曲线下面积分别为0.742、0.679、0.678、0.751、0.709,平均值鉴别两肿瘤的敏感度、特异度分别为73.3%和72.4%;偏度为60.7%和60.0%;第10百分位数为70.0%和60.7%;第50百分位数为63.3%和78.6%;第90百分位数为76.7%和67.9%.结论 ADC直方图分析可以提供更多量化信息特征,为鉴别胶质母细胞瘤和脑单发转移瘤提供了新的方法.

Methods Thirty cases of gLiobLastoma and 28 cases of soLitary metastatic brain tumor,pathoLogicaLLy confirmed at the First AffiLiated HospitaL of Zhengzhou University between May 2013 and October 2017,were retrospectiveLy anaLyzed. ResuLts Among the characteristics of the 9 parameters extracted from histogram,the differences in the mean,skewness,the 10th,50th,and 90th percentiLes between the two groups were statisticaLLy significant(P<0.05). The areas under the ROC curve were 0.742,0.679,0.678,0.751,and 0.709,respectiveLy. According to the identification for the two tumors by the mean,the sensitivity and the specificity were 73.3% and 72.4%,respectiveLy;the skewness was 60.7% and 60.0%;the 10th percentiLe was 70.0% and 60.7%;the 50th percentiLe were 63.3% and 78.6%;and the 90th percentiLe was 76.7% and 67.9%,respectiveLy. ConcLusion The ADC histogram anaLysis can provide more characteristics of quantitative information,and therefore represents a new method for the identification of gLiobLastoma and soLitary metastatic brain tumor.
