摘要目的 探求发育性髋关节发育不良患儿(development dysplasia of hip,DDH)和对照组儿童血小板源生长因子A(PDGF-A)在关节囊中的免疫组化分布规律与水平差异,并比较PDGF-A 在mRNA水平的表达差异,以探索髋关节松弛的原因.方法 共收集了髋脱位患儿64例和对照组儿童14例,二组之间成功配对8组,进行配对研究,每组配对条件为性别相同、年龄相近,采用S-P法免疫组化技术和半定量RT-PCR技术检测关节囊中PDGF-A的分布规律与水平差异,以及在mRNA水平的差异.运用图像分析软件进行量化分析,并采用SPSS 10.0软件进行统计学分析.结果 可见分泌PDGF-A的成纤维细胞于关节囊内贴近关节侧的滑膜层呈强阳性表达,内部纤维层阳性表达细胞稀疏,强度明显减弱.8例患儿与对照组配对比较,纤维层中有阳性表现的成纤维细胞数占成纤维细胞总数的百分比(配对T检验P＜0.001)和局部染色的灰度值(配对T检验P＜0.05)在二组间差异存在统计学意义,DDH组关节囊PDGF-A mRNA的表达较对照组减少(配对T检验P＜0.01).结论 PDGF-A的分布减少和表达异常很可能与DDH患儿的髋关节松弛有关.

abstractsObjective To study the expression of platelet-derived growth factor-A (PDGF-A) in the hip capsule of children with developmental dysplasia of the hip (DDH).Methods The clinical data of 64 patients with DDH and 14 control children were retrospectively analyzed.Among them,8 pairs of DDH patients and their controls were chosen for this study.The tissues were collected during surgery.The PDGF-A in the hip capsule was detected using immunohistochemistry (SP method) staining.RT-PCR was used to study the mRNA expression of PDGF-A.Results High expression of PDGF-A was observed in the synovial layer with fibroblast regularly arranged parallel to the joint surface.PDGF-A expression was decreased in the fibrous layer of the hip capsule.The percentage of positive fibroblast and gray-scale density was significantly different in the fibrous layer of the DDH patients compared to controls (P＜0.05).The mRNA expression of PDGF-A was also decreased in the DDH patients compared to the controls (P＜0.01).Conclusions The decreased expression of PDGFA in the hip capsule may involve in the development of DDH.
