文化能力培训与卫生保健质量改善
Linking cultural competence training to improved health outcomes: perspectives from the field
摘要文化能力是临床医疗的重要组成部分,是保障医师职业化水平与提供高质量服务不可或缺的技能之一.有关证据表明,跨文化培训(又称文化能力培训)确能改进医生的态度、知识与技能,并使患者接受到高质量的服务,但培训对改善卫生保健质量起到的作用尚无论证.本文讨论了文化能力培训的发展及其原则,从实践经验出发,总结了具有现实指导意义的培训注意事项;提出了一个详尽的框架体系与检测方案,用以评估培训对卫生保健质量改善所起的作用.最后指出,文化能力培训应当参照其他教学活动的衡量标准,运用卫生科研部门研发的工具和质量改进原则对培训活动效果逐步加以评估.

abstractsCultural competence represents an important building block of clinical care,as well as a skill set that is central to professionalism and quality.Although evidence shows that cultural competence training improves the attitudes,knowledge,and skill of physicians as well as patients' ratings of care,no definite evidence has yet linked this training to improved health outcomes.The paper started from the evolution and key principles of cultural competence training and provided us with the lessons about how to ensure a successful training.The focus of the paper was the creation of an evaluation framework and measurement that might be effective in identifying the impact of cultural competence training on improved health care outcomes.Cultural competence training should be evaluated in a stepwise fashion by using the tools of health services research and the principles of quality improvement,and it should be held to the same standards as other educational interventions and activities.
