摘要目的 比较口腔医学专业学生在儿童口腔临床实习方式改革前后的实习效果.方法 以北京大学2003级和2004级八年制口腔医学专业80名学生为研究对象,其中,2003级39名学生为对照组,2004级41名学生为实验组.对照组采用传统临床实习方式进行实习,实验组采用改革的临床实习方式进行实习.改革的临床实习方式为:缩短了接诊儿童患者时间,增加接诊年轻的牙体牙髓患者,增加外伤病例PBL教学等.教学结束后,比较实习工作量登记手册中两组学生的操作数量并就实习方式对学生进行了问卷调查.结果 实验组完成乳恒牙龋齿充填,乳恒牙牙髓治疗和外伤病例治疗等儿童口腔基本临床操作的病例数同对照组学生相比没有减少(P>0.05).但是,如果加上平时接诊年轻牙体牙髓病患者进行的恒牙龋齿充填和牙髓治疗的操作,实验组完成这两种操作的病例数显著多于对照组(P<0.05).问卷结果显示,实验组学生肯定了改革的儿童口腔临床实习方式,对照组大部分学生要求在实习中增加病源,增加实践机会的愿望强烈.结论 在儿童口腔临床实习患者就诊量减少的情况下,改革的儿童口腔临床实习方式更能合理分配实习时间,增加临床实践机会,提高临床实习效果.

abstractsObjective The efficacy of the pediatric clinical practice was compared between the reformed practice model and the traditional practice model.Methods Using 80,Eight-year program graduate dentists between 2003 and 2004 as the object of research,and the students of grade 2003 ( n =39) for control group and that of grade 2004( n=41 ) for experimental group.Clinical practice of control group was arranged in the traditional pediatric dental practice model.Clinical practice of experimental group was organized by shortening the practice time of treating child patients,admitting treatment of adolescent endodontic patients and adding problem-based learning( PBL) of dental trauma case to the teaching schedule.At the end of the clinical practice,the number of clinical operation record and the questionnaire survey about the practice models among the students were compared.Results The numbers of caries fillings,pulp therapies and dental-trauma cases of child patients in experimental group was not statistically decreased compared to control group (P>0.05).But the number of pulp therapies and caries fillings in permanent teeth of all patients in experimental group overwhelmed the counterparts of only child patients in control group (P<0.05).Questionnaire-survey revealed that dental students in experimental group appreciated the innovation very much,and dental students in control group expected more patient admission and practice opportunities.Conclusions Under the condition of limited out-patient numbers in pediatric dentistry,by applying the reformed practice model,the time is more reasonably distributed,practice opportunity is increased and the effect of practice is improved.
