视锥细胞营养不良和Stargardt's病多焦视网膜电图潜伏期的差异
Preliminary observations of the differences between multifocal electroretinograms in cone dystrophy and Stargardt's disease
摘要目的:观察视锥细胞营养不良和Stargardt's病多焦视网膜电图潜伏期的差异.方法:用Veris Ⅳ系统记录分析4例视锥细胞营养不良、4例Stargardt's病和17例正常对照眼的多焦视网膜电图.将两组患者的结果分别与其年龄匹配的正常对照组的结果进行比较.结果:与正常对照组的结果比较,两组患者的多焦视网膜电图的反应密度在1~6环均有显著性下降.两组患者的反应密度在视野中央呈较明显下降,随离心度增加反应密度逐渐趋近正常对照范围.视锥细胞营养不良组的潜伏期在1~3环和5~6环比正常对照组延长,但只有5~6环的数据与正常对照组差异有显著性.Stargardt's病组的潜伏期在视野中央比正常对照组延长,但只有3~4环的数据差异有显著性.结论:在较大离心度的视野部位,视锥细胞营养不良组的潜伏期比Stargardt's病组的潜伏期长,因此多焦视网膜电图的潜伏期有可能作为鉴别这两种遗传性眼底病的指标之一.

abstractsObjective:To make preliminary observations comparing the differences in the characteristic changes of multifocal electroretinograms(mfERG) in progressive cone dystrophy and Stargardt's disease.Methods:The latencies and response densities of mfERG were measured in four patients with progressive cone dystrophy(4 left eyes),four patients with Stargardt's disease(4 left eyes) and 17 normal control subjects(17 left eyes) using the veris Ⅳ system. Age-matched data were compared for each patient group and the normal control group.Results:Compared with normal controls,both cone dystrophy and Stargardt's disease showed significant attenuation in response densities in ring 1 to ring 6. The differences in response densities between patient groups and the normal control group were reduced with the increase in eccentricity from ring 1 to ring 6. However,latencies revealed different changes for cone dystrophy and Stargardt's disease. In cone dystrophy,compared with normal controls,latencies were relatively longer from ring 1 to ring 3 and from ring 5 to ring 6. Statistical analysis showed that the latencies were significantly prolonged only in ring 5 and ring 6 because ring 1 to ring 3 had larger standard deviations in latencies. However,in Stargardt's disease,latencies were relatively longer in the central rings compared with normal controls. There was a decrease in latency prolongation with an increase in eccentricity. Due to the marked attenuation of the response densities in the central region,the standard deviations of the latencies were greater,so latencies were significantly prolonged only in ring 3 and ring 4.Conclusion:The eccentricities of the loci for latency prolongation in subjects with cone dystrophy are larger than for those with Stargardt's disease. In conclusion,mfERG will probably be one of the methods used in the future for differentiating cone dystrophy from Stargardt's disease.
