基于OCT对帕金森病患者视网膜外丛状层影像特征的初步研究

Quantitative Evaluation of Image Features in the Retinal Outer Plexiform Layer of Parkinson's Disease by OCT

摘要目的::定量评估帕金森病(PD)患者视网膜外丛状层(OPL)的OCT影像特征的变化。方法::横断面研究。连续性纳入2022年11月至2023年3月在温州医科大学附属眼视光医院确诊的PD患者59例(59眼),同期纳入性别和年龄匹配区间随机抽样的健康对照组35例(35眼)。利用OCT对所有受检者黄斑区进行放射状扫描,获取黄斑中心直径3 mm区域的OPL影像特征。采用 t检验比较PD组和对照组的OPL影像特征的差异性,并采用Pearson和Spearman相关分析评估OPL影像特征与临床指标之间的相关性。 结果::PD患者的黄斑中心3 mm区域OPL厚度为(11.22±3.95)μm,较健康对照组(9.37±1.07)μm有明显增加( t=-3.36, P=0.001)。PD组和健康对照组间多个影像特征发生显著改变,其中逆差分归一化(IDN)特征值增大、运行长度非均匀归一化(RLNN)和运行百分比(RP)特征值减小,差异均有统计学意义(均 P<0.001)。OPL的厚度和IDN与统一帕金森病评价量表(UPDRS) ( r=0.24, P=0.019; r=0.61, P<0.001)、UPDRS III评分( r=0.27, P=0.010; r=0.62, P<0.001)、病程( r=0.26, P=0.013; r=0.66, P<0.001)、Hoehn-Yahr评分( r=0.28, P=0.006; r=0.70, P<0.001)均呈正相关。RLNN和RP与UPDRS评分( r=-0.62、-0.61;均 P<0.001)、UPDRS III评分( r=-0.62、-0.61;均 P<0.001)、病程( r=-0.68、-0.67;均 P<0.001)、Hoehn-Yahr评分( r=-0.69、-0.69;均 P<0.001)均呈负相关。 结论::PD患者视网膜黄斑区域的OPL厚度增加,IDN特征值增大,RLNN和RP特征值减小,且与Hoehn-Yahr评分、病程、UPDRS评分和UPDRS III评分相关。

abstractsObjective::To quantity analyze the image features including the thickness and texture in the retinal outer plexiform layer (OPL) of Parkinson's disease (PD) patients by optical coherence tomography (OCT).Methods::A cross-sectional study was conducted in the Eye Hospital of Wenzhou Medical University from November 2022 to March 2023. A total of 59 consecutive eyes of 59 patients with PD were enrolled as PD group and 35 eyes of 35 age-and gender-matched healthy subjects were enrolled in the healthy control group. OCT was used to obtain the image features within 3 mm diameter area around the macula through circular scanning at the macula of all subjects. The differences in OPL image features between PD and healthy control groups were compared by t-test, and the correlation of mean values from selected features with the clinical measures were evaluated by Pearson and Spearman correlation analysis. Results::The OPL thickness of PD patients was 11.22±3.95 μm at 3 mm area around the macula, which was evidently higher than 9.37±1.07 μm in the healthy control group ( t=-3.36, P=0.001). Multiple texture features between the two groups were significantly differed between the PD group and healthy control group, the inverse difference normalized (IDN) feature value increased, while values of the run length non-uniformity normalized (RLNN) and run percentage (RP) features decreased (all P<0.001). OPL thickness and IDN was positively correlated with Unified Parkinson's Disease Rating (UPDRS) score ( r=0.24, P=0.019; r=0.61, P<0.001), UPDRS III score ( r=0.27, P=0.010; r=0.62, P<0.001), disease duration ( r=0.26, P=0.013; r=0.66, P<0.001) and Hoehn-Yahr score ( r=0.28, P=0.006; r=0.70, P<0.001). RLNN and RP were negatively correlated with UPDRS score ( r=-0.62, -0.61, both P<0.001), UPDRS III score ( r=-0.62, -0.61, both P<0.001), disease duration ( r=-0.68, -0.67, both P<0.001) and Hoehn-Yahr score ( r=-0.69, -0.69, both P<0.001). Conclusions::The macular OPL thickness of PD patients increased, and the IDN feature value increased, while values of the RLNN and RP features decreased, which were correlated with Hoehn-Yahr score, disease duration, UPDRS and part III score.
