摘要目的 探讨流式细胞术检测HLA-B27在临床上诊断强直性脊柱炎(AS)的价值.方法 采用流式细胞术对75例确诊为AS患者(AS组)、95例疑为AS患者(非AS组)和45例健康体检者(对照组)外周血进行HLA-B27表达的检测.结果 AS组HLA-B27荧光强度和阳性率明显高于非AS组和健康对照组[158.0±24.0比94.8±30.5和89.1±18.9、93.33%(70/75)比5.26%(5/95)和4.44%(2/45)],差异有统计学意义(P<0.05).HLA-B27检测在诊断AS的敏感度为93.3%(70/75),特异度为94.7%(90/95),诊断正确率为94.1%(160/170). 结论 HLA-B27与AS高度相关,流式细胞术检测HLA-B27在AS的早期诊断和鉴别诊断中起着重要作用.

abstractsObjective To investigate the clinical value of HLA-B27 testing in diagnosis of ankylosing spondylitis (AS) by flow cytometry. Methods The peripheral blood HLA-B27 was detected by flow cytometry in 75 diagnosed AS patients (AS group), 95 suspected AS patients (non-AS group) and 45 health people (control group). Results The fluorescence intensity and the positive rates of HLA-B27 in AS group were significantly higher than those in non-AS group and control group: 158.0 ±24.0 vs. 94.8 ±30.5 and 89.1 ±18.9, 93.33% (70/75) vs. 5.26% (5/95) and 4.44% (2/45), and there were statistical differences (P<0.05). The sensitivity of HLA-B27 in diagnosis of AS was 93.3% (70/75), the specificity was 94.7%(90/95), while the accuracy was 94.1%(160/170). Conclusions There is a high correlation between AS and HLA-B27. The flow cytometry assay of HLA-B27 plays an important role in the early diagnosis and differential diagnosis of AS.
