急性心肌梗死患者血清长链非编码RNA OIP5-AS1和微小RNA-363-3p表达与心血管事件发生的相关性研究

Correlation of serum long non-coding RNA OIP5-AS1 and microRNA-363-3p expression with cardiovascular events in patients with acute myocardial infarction

摘要目的:探讨长链非编码RNA(lncRNA)OIP5-AS1、微小RNA(miR)-363-3p与急性心肌梗死(AMI)患者心血管事件发生的相关性。方法:回顾性选取2020年1月至2021年12月恩施土家族苗族自治州中心医院收治的130例AMI患者,治疗后通过门诊或电话形式对AMI患者进行为期1年的随访,记录是否出现心血管事件,将发生心血管事件的患者纳入发生心血管事件组(55例),反之纳入未发生心血管事件组(75例);另选择同期60例健康体检者作为健康对照组。采用实时荧光定量聚合酶链反应法检测三组血清lncRNA OIP5-AS1、miR-363-3p表达水平;采用Pearson检验分析lncRNA OIP5-AS1、miR-363-3p表达水平的相关性;采用受试者工作特征(ROC)曲线分析血清lncRNA OIP5-AS1、miR-363-3p表达水平对AMI患者心血管事件发生的预测价值;采用Logistic回归分析影响AMI患者心血管事件发生的危险因素。结果:发生心血管事件组和未发生心血管事件组血清lncRNA OIP5-AS1表达水平低于健康对照组(0.41 ± 0.09、0.62 ± 0.15比1.00 ± 0.23),而发生心血管事件组血清lncRNA OIP5-AS1表达水平低于未发生心血管事件组,差异均有统计学意义( P<0.05)。发生心血管事件组和未发生心血管事件组血清miR-363-3p表达水平高于健康对照组(2.33 ± 0.58、1.78 ± 0.45比1.02 ± 0.28),而发生心血管事件组血清miR-363-3p表达水平高于未发生心血管事件组,差异均有统计学意义( P<0.05)。Pearson检验结果显示,AMI患者血清miR-363-3p表达水平与lncRNA OIP5-AS1表达水平呈负相关( r = - 0.476, P<0.05)。ROC曲线分析结果显示,血清lncRNA OIP5-AS1、miR-363-3p表达水平联合预测AMI患者发生心血管事件的曲线下面积(AUC)为0.921(95% CI 0.861 ~ 0.961),灵敏度为92.73%,特异度为81.33%,均高于单一指标检测。Logistic回归分析结果显示,lncRNA OIP5-AS1、miR-363-3p、左室射血分数均是影响AMI患者发生心血管事件的独立危险因素( P<0.05)。 结论:AMI患者血清lncRNA OIP5-AS1表达降低,miR-363-3p表达升高,两者表达水平与心血管事件发生密切相关,可作为预测AMI患者发生心血管事件的血清学指标。

abstractsObjective:To investigate the correlation between long non-coding RNA (lncRNA) OIP5-AS1 and microRNA (miR)-363-3p and cardiovascular events in patients with acute myocardial infarction (AMI).Methods:A total of 130 AMI patients who treated in the Central Hospital of Enshi Tujia and Miao Autonomous Prefecture from January 2020 to December 2021 were enrolled, after followed up for 1 year, cardiovascular event occurred in 55 patients (cardiovascular event group), and 75 patients was in non-cardiovascular event group. Meantime, 60 healthy subjects were regarded as the normal control group. The expression levels of lncRNA OIP5-AS1 and miR-363-3p in serum were detected by real-time fluorescence quantitative polymerase chain reaction. Pearson method was used to analyze the correlation of lncRNA OIP5-AS1 and miR-363-3p expression. Receiver operating characteristic (ROC) curve was used to analyze the predictive value of serum lncRNA OIP5-AS1 and miR-363-3p level on cardiovascular events in patients with AMI. Logistic regression was used to analyze the factors affecting the occurrence of cardiovascular events in patients with AMI.Results:The expression level of lncRNA OIP5-AS1 in the cardiovascular event group and non-cardiovascular event group was lower than that in the normal control group: 0.41 ± 0.09, 0.62 ± 0.15 vs. 1.00 ± 0.23, and the expression level of lncRNA OIP5-AS1 in the cardiovascular event group was lower than that in the non-cardiovascular event group, there were statistical differences ( P<0.05). The expression level of miR-363-3p in the cardiovascular event group and non-cardiovascular event group was higher than that in the normal control group: 2.33 ± 0.58, 1.78 ± 0.45 vs. 1.02 ± 0.28, and the expression level of miR-363-3p in the cardiovascular event group was higher than that in the non-cardiovascular event group, there were statistical differences ( P<0.05). Pearson method analysis showed that the expressions of miR-363-3p and lncRNA OIP5-AS1 in serum of AMI patients was obviously negatively correlated ( r = - 0.476, P<0.05). The ROC curve analysis results showed that the area under the curve (AUC) of serum lncRNA OIP5-AS1 and miR-363-3p levels joint in predicting cardiovascular events in AMI patients was 0.921(95% CI 0.861 - 0.961), the sensitivity was 92.73%, and the specificity was 81.33%, and they were higher than single index. Logistic regression analysis showed that lncRNA OIP5-AS1, miR-363-3p and left ventricular ejection fraction were independent risk factors for cardiovascular events in patients with AMI ( P<0.05). Conclusions:The expression of lncRNA OIP5-AS1 in serum of patients with AMI is decreased, and the expression of miR-363-3p is increased. The expressions of the two index are closely related to the occurrence of cardiovascular events, and may be used as serological indicators to predict the occurrence of cardiovascular events in patients with AMI.
