佩戴义眼者结膜囊菌群状况分析
Analysis of the bacterial flora in conjunctival sac of patients with ocular prosthesis
摘要目的 分析佩戴义眼的结膜囊与对侧健眼结膜囊菌群状况、药物敏感性及义眼结膜囊细菌相关影响因素.方法 对佩戴义眼的结膜囊分泌物增多的患者51例询问连续佩戴义眼时间,是否每天清洗,使用抗生素滴眼液情况,采取义眼和对侧健眼的结膜囊分泌物分别作细菌培养及药物敏感试验.应用多元Logistic回归分析方法对数据进行统计学处理.结果 佩戴义眼的结膜囊细菌阳性率为70.59%(36/51).对侧健眼(42只眼)结膜囊细菌阳性率为7.14% (3/42).佩戴义眼的结膜囊内主要细菌为金黄色葡萄球菌、表皮葡萄球菌和铜绿假单胞菌.大多数细菌对头孢吡肟、头孢哌酮(舒巴坦)、哌拉西林(他唑巴坦)、阿米卡星敏感.统计学分析义眼结膜囊细菌与是否使用抗生素滴眼液和连续配戴义眼时间长短有关.结论 革兰氏阳性球菌仍然是义眼结膜囊分离出的主要菌属,有铜绿假单胞菌的患者健侧眼手术时要注意预防眼内炎的发生.

abstractsObjective To analyze the bacterial flora and antibiotic sensitivity in the conjunctival sac of the ocular prosthesis and that of contralateral healthy eye, and the relative factors of bacterial flora in the conjunctival sac of the ocular prosthesis.Methods In this study, 51 patients who wore the ocular prosthesis with increased secretions were asked how long they wore it and whether they rinsed it everyday with or without antibiotic eyedrop, then the secretions of both conjunctival sac were accepted bacterial culture and antibiotic sensitivity testing.Multivariate logistic regression analysis was used tor data processing.Results The positive rate of bacterial culture in ocular prosthesis was 70.59% (36/51).The positive rate of bacterial culture in contralateral eyes was 7.14% (3/42).The main bacteria in conjunctival sac of ocular prosthesis were staphylococcus aureus, staphylococcus epidermidis and pseudomonas aeruginosa.Most of the bacteria were sensitive to cefepime, cefoperazone-sulbactam, piperacillin-tazobactam and amikacin.The bacterial flora of conjunctival sac in ocular prosthesis was related to how long they wore it and whether they used antibiotic eyedrop.Conclusion Gram-positive cocci is still the predominant species of bacteria in conjunctival sac of ocular prosthesis.The patients with pseudomonas aeruginosa in conjunctival sac of ocular prosthesis during the surgery of contralateral eye should be taken attention to prevent the occurrence of endophthalmitis.
