磁共振三维成像在高度近视后巩膜加固手术前后的应用
The application of three-dimensional MRI for posterior sclera reinforcement of patients with high myopia
摘要目的 观察磁共振三维成像在高度近视后巩膜加固术(PSR)的应用和作用.方法 19例(20只眼)高度近视行PSR.患者术前及术后均行磁共振三维成像序列检查,术前测量上下角膜缘通过后巩膜葡萄肿的弧长、后巩膜葡萄肿的水平弧长、后巩膜葡萄肿的垂直弧长、视神经颞侧缘至后巩膜葡萄肿颞侧缘的弧长,术后重建手术条带的影像.结果 磁共振三维成像可以获得与后巩膜葡萄肿相关的三维测量值.PSR条带显示清晰.条带恰好加固于葡萄肿的有14只眼,条带偏离葡萄肿的有6只眼,其中5只眼条带偏向颞侧,1只眼条带紧邻视神经,经二次手术后条带位置适中.结论 磁共振三维成像有助于PSR术前评估及术后评价,具有一定的临床应用价值.

abstractsObjective To observe the application and function of three-dimensional magnetic resonance imaging (3-D MRI) for posterior sclera reinforcement (PSR) of patients with high myopia.Methods Twenty eyes of 19 cases with high myopia underwent PSR.All cases took 3-D MRI before and after surgery.Before surgery,the arc length from upper corneal limbus through the peak of the scleral staphyloma to under corneal limbus,horizontal and vertical arc length of the staphyloma,and arc length from temporal border of the staphyloma to optical nerve were measured.In postoperative PSR,the image of strip was reconstructed.Results The three dimensional measured value related to the staphyloma was obtained by 3-D MRI.PSR strip showed clear.Among all cases,the strips just reinforcement in the staphyloma were 14 eyes.Six eyes occurred deviation from the staphyloma,including 5 eyes deviated to temporal side of the staphyloma and 1 eye closed to the optic nerve.After the second times of the PSR,the position of the strip was moderate.Conclusion The application of 3-D MRI contributes to preoperative assessment and postoperative evaluation,it has the certain value in clinical application.
