伴高度近视的白内障测算人工晶状体屈光度的准确性
Accuracy of intraocular lens diopter calculation for high myopia with cataract
摘要目的 比较伴高度近视的白内障患者手术前所需人工晶状体(IOL)3种测算公式的准确性.方法 回顾性分析2017年2月至2018年11月该院伴高度近视的白内障172例(172眼)的临床资料.根据眼轴长度分为3组:A组,眼轴为26.1~28.0 mm;B组,眼轴为28.1~30.0 mm和C组,眼轴为≥30.1 mm.3组分别用SRK/T、Haigis和Barrett Universal Ⅱ 3种不同的公式测算所需人工晶状体的屈光度.术后1个月对比准确性三公式人工晶状体屈光度的准确性.结果 A组分别以3个公式测算的人工晶状体屈光度准确性差异无统计学意义(P=1.000),B组和C组测算所需人工晶状体的屈光度以Barrett Universal Ⅱ公式最佳.术后患者实际屈光度与预期屈光度误差在±0.5D范围者Barrett Universal Ⅱ公式占比最高(70.9%),差异有统计学意义(x2=18.801,P=0.000);屈光误差在±1.0D的占比3公式间差异无统计学意义(x2=5.007,P=0.082).结论 Barrett Universal Ⅱ公式对于伴高度近视的白内障所需IOL的屈光度测算较SRK/T和Haigis公式更为准确.

abstractsObjective To compare the calculation accuracy with three different intraocular lens (IOL) diopter formulas for high myopia with cataract.Methods The data of 172 eyes of 172 patients with high myopia with cataract from Feb.2017 to Nov.2018 in this hospital were analyzed retrospectively.All cases were divided into 3 groups according to axial length (AL).AL of group A was 26.1-28.0 mm,28.1-30.0 mm in group B and ≥30.1 mm in group C.The intraocular lens diopter of three groups were calculated with SRK/T,Haigis and Barrett Universal Ⅱ formulas,respectively.The accuracy of IOL diopter among three formulas were compared in each groups at 1 month postopeeratively.Results The difference of IOL diopter accuracy in group A were not statistically significant among three formulas (P =1.000).The IOL diopter with Barrett Universal Ⅱ was the most accurate in group B and group C.Barrett Universal Ⅱ was the highest (70.9%) in eyes with error ≤ ± 0.5 D between actual diopter and expected diopter.The difference was statistically significant among three formulas (x2 =18.801,P=0.000).The difference in percentage of prediction error within ± 1.0 D was not statistically significant among the three formulas (x2 =5.007,P=0.082).Conclusion The calculation accuracy of IOL diopter with Barrett Universal Ⅱ formula is the highest for high myopia with cataract.
