摘要目的:评价眼轮匝肌折叠缩短术治疗退行性下睑内翻的效果。方法:前瞻性随机对照研究。纳入浙江骨伤医院2019年5月至2022年1月退行性下睑内翻70例(70眼)，按随机数字表法分为两组，每组各35例。研究组采用眼轮匝肌折叠缩短术治疗，对照组采用眼睑轮匝肌切除术治疗。术后随访3个月，比较两组临床疗效。结果:研究组有效率为94.29%(33/35)，高于对照组的74.29%(26/35)( χ2＝4.01， P<0.001)。手术后：两组患者畏光、溢泪、异物感和刺激症状评分均低于手术前，且研究组各眼部症状评分均低于对照组(均 P<0.05)。两组患者下睑牵引试验数值和眼睑侧方牵引试验、下睑复位试验阳性率均降低，且研究组均低于对照组(均 P<0.05)。泪膜破裂时间和Schirmer Ⅰ试验均较手术前升高，且研究组高于对照组(均 P<0.05)；两组患者眼睑美观满意度评分较手术前升高，两组间比较差异无统计学意义( t＝1.63， P＝0.107)。两组并发症发生率对比差异亦无统计学意义( χ2＝0.00， P＝1.000)。 结论:眼轮匝肌折叠缩短术治疗退行性下睑内翻临床效果良好。

abstractsObjective:To evaluate the efficacy of orbicularis oculi muscle fold shortening in the treatment of degenerative lower eyelid entropion.Methods:This was a prospective case control study. A total 70 eyes of 70 patients with degenerative lower eyelid entropion from May 2019 to Jan. 2022 in Zhejiang Bone Injury Hospital were included. They were divided into two groups based on random number table methods, with 35 cases in each group. Patients in the research group were treated with the eyelid orbicularis muscle folding and shortening, while those in the control group were treated with the eyelid orbicularis muscle resection. The patients were followed up for 3 months after operation and the clinical efficacy between the two groups were compared.Results:The effective rate in the research group was 94.29%(33/35), which was higher than the 74.29%(26/35) in the control group ( χ2=4.01, P<0.001). After operation: the symptom scores of photophobia, epiphora, foreign body sensation and irritation in the two groups were lower than those before operation, and the symptom scores of eyes in the research group were lower than those in the control group (all P<0.05). The values of lower eyelid traction test, the positive rate of eyelid lateral traction test and lower eyelid reduction test were decreased in both groups, and those of the research group were all lower than the control group (all P<0.05). The break-up time and Schirmer Ⅰ test were higher than those before surgery and higher in the research group than in the control group (all P<0.05). The eyelid aesthetic satisfaction scores of two groups were higher than those before surgery, and there was no difference between the two groups ( t=1.63, P=0.107). There was no difference in the incidence of complications between the two groups ( χ2=0.00, P=1.000). Conclusion:Orbicularis oculi muscle fold shortening has good clinical efficacy on degenerative lower eyelid entropion.
