复方血栓通胶囊对特发性黄斑孔患者术后视功能恢复的效果观察
Effects of Fufang Xueshuantong capsule on postoperative visual function recovery in patients with idiopathic macular hole
摘要目的:观察复方血栓通胶囊辅助治疗对特发性黄斑孔患者术后视功能恢复的效果。方法:前瞻性随机对照研究。纳入浙江大学医学院附属第二医院眼科中心于2018年12月至2022年12月实施手术治疗,且术后黄斑孔闭合的特发性黄斑孔49例(49只眼),按简单随机法分为试验组22例和对照组27例,术后两组患者均给予左氧氟沙星滴眼液和醋酸泼尼松龙滴眼液滴眼治疗,试验组加用复方血栓通胶囊治疗。观察患者术后1、3、6个月的最佳矫正视力(BCVA, logMAR),并以光学相干断层扫描成像(OCT)及光学相干断层扫描血管成像(OCTA)测量黄斑中心区视网膜厚度(CRT)、黄斑区浅层毛细血管(SCP)以及深层毛细血管丛(DCP)黄斑中心无血管区域(FAZ)的面积、脉络膜毛细血管血流面积(CBFA)。结果:试验组患者CBFA术后3个月(2.58±0.04)mm 2和6个月(2.49±0.02)mm 2,均高于对照组的(1.96±0.03)mm 2和(1.89±0.05) mm 2 ( t=3.16、2.46, P=0.023,0.043);而术后1、3、6个月,两组的BCVA、CRT、SCP和DCP的FAZ面积,差异均无统计学意义(均 P>0.05)。 结论:特发性黄斑孔患者术后采用复方血栓通胶囊辅助治疗,尽管未改善患者视力,但可更早地促进脉络膜毛细血管血流面积增加和正常化。

A total of 49 patients (49 eyes) with idiopathic macular hole who underwent surgical treatment at the eye center of the Second Affiliated Hospital, School of Medicine, Zhejiang University from Dec. 2018 to Dec. 2022 were selected. All patients had idiopathic macular hole closure after surgery. The patients were divided into the clinical trial group (22 cases) and the control group (27 cases) based on a simple random grouping method. Both groups were treated with levofloxacin eye drops and prednisolone acetate eye drops after surgery, while the clinical trial group was additionally treated with Fufang Xueshuantong. The best corrected visual acuity (BCVA, logMAR) of the patients at 1, 3, and 6 months after surgery were observed, and the central retinal thickness (CRT), area of the foveal avascular zone (FAZ) in the superficial capillary plexus (SCP) and deep capillary plexus (DCP), and the choriocapillary blood flow area (CBFA) were also measured by optical coherence tomography (OCT) and optical coherence tomography angiography (OCTA), respectively.Results:CBFA in the clinical trial group patients significantly increased at 3 months [(2.58±0.04) mm 2] and 6 months [(2.49±0.02) mm 2] after surgery compared with that in the control group [(1.96±0.03)mm 2 and (1.89±0.05) mm 2] ( t=3.16, 2.46; P=0.023, 0.043, respectively); However, there were no significant differences in BCVA, CRT, and the FAZ of SCP and DCP between the two groups at 1, 3, and 6 months after surgery ( P>0.05). Conclusion:Fufang Xueshuantong capsule can improve and normalize the postoperative choroidal blood flow in patients with idiopathic macular hole.
