摘要目的:探讨针灸联合表情肌功能康复治疗面神经麻痹的疗效观察及对患者面神经功能及面部残疾指数的影响。方法:选择浙江中医药大学附属第三医院于2018年1月至2019年1月收治的面神经麻痹患者82例，依据随机数字表法分为观察组41例与对照组41例。对照组采用常规治疗，观察组在常规治疗基础上结合针灸联合表情肌康复训练。两组疗程均为4周。比较两组治疗疗效和H-B面神经功能变化，治疗前后面部残疾指数(FDI)及瞬目反射R1和R2波潜伏期变化。结果:观察组总有效率(92.68%)高于对照组(75.61%)(χ 2＝4.479， P<0.05)；观察组治疗后H-B面神经功能分级Ⅰ级例数多于对照组(28比20)(χ 2＝4.216， P<0.05)。观察组治疗后FDIS评分[(29.71±4.23)分]和FDIP评分[(31.23±4.25)分]均高于对照组[(23.18±3.09)分和(24.31±3.56)分]( t＝7.982、7.992，均 P<0.05)。观察组治疗后瞬目反射R1波潜伏期[(12.21±1.17)s]和R2波潜伏期[(32.31±1.52)s]均短于对照组[(15.26±0.93)s和(35.64±2.01)s]( t＝13.067、8.461，均 P<0.05)。 结论:针灸联合表情肌功能康复对面神经麻痹患者疗效明显，且可改善患者面神经功能，提高患者面部残疾指数。

abstractsObjective:To explore the effect of acupuncture combined with facial muscle function rehabilitation on facial paralysis and its influence on facial nerve function and facial disability index (FDI).Methods:From January 2018 to January 2019, 82 patients with facial paralysis admitted to the Third Affiliated Hospital of Zhejiang University of Traditional Chinese Medicine were randomly divided into observation group (41 cases) and control group (41 cases) according to random number table method.The control group received routine treatment, while the observation group received acupuncture combined with rehabilitation training on the basis of routine treatment.Both two groups were treated for 4 weeks.The therapeutic effect and H-B facial nerve function were compared between the two groups, and the changes of FDI and latency of blink reflex R1 and R2 wave before and after treatment were compared.Results:The total effective rate of the observation group (92.68%) was higher than that of the control group (75.61%) (χ 2=4.479, P<0.05). The case number of H-B facial nerve function grade Ⅰ of the observation group after treatment was higher than that of the control group (28: 20, χ 2=4.216, P<0.05). The FDIS score[(29.71±4.23)points] and FDIP score[(31.23±4.25)points] in the observation group were higher than those in the control group[(23.18±3.09)points and (24.31±3.56)points]( t=7.982, 7.992, all P<0.05). The latency of R1 wave[(12.21±1.17)s] and R2 wave[(32.31±1.52)s] in the observation group were shorter than those in the control group[(15.26±0.93)s and (35.64±2.01)s]( t=13.067, 8.461, all P<0.05). Conclusion:Acupuncture combined with expressional muscle function can improve facial nerve function and facial disability.
