摘要目的 评价合并黄斑裂孔白内障超声乳化吸除术后早期临床效果.方法 对2010年3月至2012年11月在南京爱尔眼科医院就诊的31例(35只眼)合并黄斑裂孔的白内障患者超声乳化术后情况进行了观察.根据术前相干光断层扫描(OCT)检查,确定黄斑裂孔的分期,观察术后视力及术中术后并发症,分析术后视力与黄斑裂孔分期的关系.结果 全部患者术后视力较术前明显改善.术后1个月最佳矫正视力低于0.1者3只眼,0.1～＜0.3者6只眼,0.3～＜0.5者11只眼,≥0.5者15只眼.术后3个月最佳矫正视力低于0.1者2只眼,0.1～＜0.3者9只眼,0.3～＜0.5者10只眼,≥0.5者14只眼.黄斑裂孔的分期与术后视功能改善程度有相关性,差异有统计学意义(P＜0.05),黄斑裂孔所处分期越早,术后视力改善越明显.术中晶状体后囊破裂1只眼,术后角膜水肿5只眼,未见视网膜脱离.结论 伴黄斑裂孔白内障患者术后视力较术前明显提高,术后视力的提高程度与黄斑裂孔的分期明显相关,OCT检查能够有助于预测术后视力.

abstractsObjective To evaluate early clinical outcomes of phacoemulsification and intraocular implantation for patients combined idiopathic macular hole.Methods Thirty one cases(35 eyes) were analyzed for patients with phacoemulsification and lens implantation combined idiopathic macular hole.All of them preoperative examinations included optical coherence tomography(OCT),according to Gass grades,stage of idiopathic macular hole was definitive.Best correct visual acuity after surgery,complication,also relation between visual acuity and stage of idiopathic macular hole were observed.Results Three months postoperatively,the best corrected visual acuity in all cases had improved in different degrees.BCVT was found after surgery in a month,＜0.1 three eyes,0.1～＜0.3 six eyes,0.3～＜0.5 eleven eyes,≥0.5 fifteen eyes.BCVT was found after surgery in three months,＜0.1 two eyes,0.1～＜0.3 nine eyes,0.3～＜0.5 ten eyes,≥0.5 fourteen eyes.Postoperative visual acuity for cataract combined idiopathic macular hole correlated with macular hole grade,the earlier stages of idiopathic macular hole,the better postoperative visual acuity.Early postoperative complications included intraoperative posterior capsular rupture in 1 eye and corneal edema in 5 eyes.No retinal detachment was found.Conclusions Postoperative visual acuity for cataract combined idiopathic macular hole correlates with macular hole grade,and OCT is a effective method for forecasting postoperative visual acuity.
