摘要目的 探讨血清白细胞介素17 (IL-17)在类风湿性关节炎(RA)患者中的表达及其与RA病情活动的相关性.方法 75例RA患者(稳定期19例,活动期56例)和75名健康对照者采用酶联免疫吸附试验检测血清IL-17的浓度.结果 RA患者血清中IL-17水平为(28.51±11.27) ng/L,正常对照组为(22.17±5.48) ng/L,两组比较差异有统计学意义(t=4.40、P＜0.001)；活动期RA患者血清IL-17浓度为(34.16±14.42)ng/L,稳定期为(18.27±7.88) ng/L,两者比较差异有统计学意义(t=4.56、P ＜0.001)；采用Spearman相关分析,病例组患者DAS28评分与IL-17具有明显的相关性(P＜0.05).与治疗前比较,25例RA患者接受免疫抑制剂治疗后血清IL-17、DAS28、类风湿因子(RF)均有不同程度的下降[IL-17:(28.51 ±11.27)、(15.15±8.32) ng/L,t =4.77、P＜0.001;DAS28:4.30±1.29、2.35±1.11,t=3.80、P＜0.001；RF:(47.84±16.50)、(28.18±8.90) kU/L,=5.24、P＜0.001].结论 RA患者血清IL-17存在高表达,且与RA患者的DAS28评分呈正相关,IL-17可能成为RA观察疗效的一种指标.

abstractsObjective To investigate the expression of interleukin-17 (IL-17) in rheumatoid arthritis (RA) and correlations with disease activity.Methods Seventy-five RA patients including 19 cases remissive,56 cases active and 75 cases normal controls were enrolled in this study.Enzyme linked immunosorbent assay (ELISA) was applied to detect IL-17 in the sera from RA patients and normal controls.Results The level of IL-17 in RA patients was higher than that in the normal ones and the difference has statistical significance ((28.51 ± 11.27) ng/L vs.(22.17 ± 5.48) ng/L,respectively,t =4.40,P ＜ 0.001) ; The IL-17 level of in active RA patients was also higher than that of remissive ones ((34.16 ± 14.42) ng/L vs.(18.27 ± 7.88)ng/L,respectively,t =4.56,P ＜ 0.001).The relationship between DAS28 score and IL-17 level were evaluated using Spearman rank correlation test,and there was significant associations (P ＜ 0.05).25 patients with RA receiving immunosuppressive therapy,IL-17,DAS28,rheumatoid factors (RF) were significantly lower at posttreatment than which of pre-therapy (IL-17:(28.51 ± 11.27) ng/L vs.(15.15 t 8.32) ng/L,t =4.77,P ＜0.001;DAS28:(4.30 ± 1.29)vs.(2.35 ±1.11),t =3.80,P＜0.001;RF:(47.84 ± 16.50)kU/L vs.(28.18 ± 8.90) kU/L,t =5.24,P ＜ 0.001).Conclusion Levels of IL-17 was found be high in sera of RA patients and positive correlation with DAS28.It is indicated that IL-17 is a useful marker for evaluation of therapeutic effects and prognosis.
