外旋角度测量器与外旋拨号试验评估膝关节后外旋转不稳定
Evaluation of external rotation angle of the knee using an external rotation measuring-scale
摘要目的通过使用新发明的外旋角度测量器与外旋拨号试验对患者膝关节外旋角度进行测量,探讨测量膝关节外旋角度的新方法。方法 2009年11月至2010年5月连续收治20例膝关节后外复合体(PLC)损伤或PLC损伤重建术后1年取出内固定的患者,男15例,女5例;平均年龄32.9岁(20 ~41岁)。PLC损伤的诊断结合患者病史、物理查体、应力X线片及MRI检查。受伤至手术时间平均为309 d(7 d至10年)。分别使用膝关节外旋角度需测量器与外旋拨号试验对患者膝关节外旋角度进行测量,并对两种测量方法的准确性、所需时间及所需操作人数进行比较与分析。 结果 20例患者使用外旋角度测量器测量患侧与健侧外旋角度差值平均为8.6°±6.6°,外旋拨号试验测量患侧与健侧外旋角度差值平均为8.2°±7.7°,二者比较差异无统计学意义(t=0.940,P=0. 370),但外旋角度测量器的变异系数(0.77)小于外旋拨号试验(0.94),且使用外旋角度测量器所需的操作人数(2人)和所需时间[(26.1±3.9)s]明显少于外旋拨号试验[3人,(131.9±12.7)s],两种方法测量所需时间比较差异有统计学意义(t=-69.393,P<0.001)。 结论外旋角度测量器可以作为膝关节外旋角度的测量方法,其测量外旋角度的侧-侧差值与外旋拨号试验无明显差异,且较外旋拨号试验变异更小,更加节省时间和人力。

abstractsObjectiveTo report a novel measurement of external rotation angles of the knee with our self-designed external rotation measuring-scale in comparison of the dial test.MethodsBetween November 2009 and May 2010, 20 patients with injury to the posterolateral corner (PLC) of the knee were diagnosed and treated at our department. They were 15 men and 5 women, aged from 20 to 41 years (average,32. 9 years) . Their PLC injuries were diagnosed on the basis of case history, physical examination, stress X-ray and MRI. The interval between injury to surgery averaged 309 days (from 7 days to 10 years) . Their external rotation angles of the knee were measured by both the dial test and our self-designed external rotation measuring-scale to compare the 2 methods in terms of accuracy, time consumed, and operators needed. ResultsThere was no significant difference between the mean side-to-side difference by the external rotation measuring-scale (8.6° ± 6. 6°) and that by the dial test (8.2° ± 7.7°) ( t =0. 940, P =0. 370) . But the coefficient of variation by our external rotation measuring-scale (0. 77) was smaller than that by the dial test (0. 94) . The time consumed and operators needed by our external rotation measuring-scale (26. 1 ± 3.9 seconds and 2 persons) were significantly less than those by the dial test (131.9 ± 12. 7 seconds and 3 persons) ( t =- 69. 393, P < 0. 001 ).ConclusionIn measurement of external rotation angles of the knee,our self-designed external rotation measuring-scale can be as accurate as the dial test, but better than the latter in coefficient of variation, time and manpower needed.
