吗啉硝唑联合高压氧治疗Wagner 2~4级糖尿病足创面感染的回顾性队列研究
Morinidazole combined with hyperbaric oxygen therapy in the treatment of Wagner grade 2-4 diabetic foot ulcer infection : a retrospective Cohort study
摘要目的:评价吗啉硝唑联合高压氧对Wagner 2~4级糖尿病足创面感染生物标志物水平的影响及临床疗效。方法:回顾性分析2021年1月至2023年11月复旦大学附属金山医院收治的89例Wagner 2~4级糖尿病足患者的临床资料。按照治疗方法的不同,将患者分为观察组(46例)和对照组(43例)。对照组单纯采用吗啉硝唑治疗,观察组采用吗啉硝唑联合高压氧治疗,连续治疗3周后,比较2组治疗前后以及2组间的炎症指标、厌氧菌清除率、临床总有效率以及不良反应发生率。结果:治疗前,2组间炎症指标[全血白细胞计数(WBC)、血清C反应蛋白(CRP)、降钙素原(PCT)、白细胞介素-6(IL-6)]差异均无统计学意义(均 P>0.05);治疗后,2组炎症指标均低于治疗前(均 P<0.05),且观察组水平均低于对照组(均 P<0.05)。治疗后,观察组厌氧菌清除率为95.65%,对照组为74.42%,差异有统计学意义( P<0.05);观察组临床总有效率为97.83%,对照组为81.40%,差异有统计学意义( P<0.05);观察组不良反应发生率为8.70%,对照组为6.98%,差异无统计学意义( P>0.05)。 结论:吗啉硝唑联合高压氧治疗Wagner 2~4级糖尿病足的效果确切,比单用吗林硝唑药物治疗能更有效地控制糖尿病足创面感染,促进糖尿病足的愈合。

abstractsObjective:To evaluate the effect of morinidazole combined with hyperbaric oxygen therapy on the levels of infection biomarkers and clinical efficacy of Wagner grades 2–4 diabetic foot ulcer infection.Methods:The clinical data of 89 selected patients with Wagner grades 2–4 diabetic foot ulcer , hospitalized in our hospital from January 2021 to November 2023 , were retrospectively analyzed. The patients were divided into observation group(46 cases ) and control group(43 cases ) based on different treatment methods. The control group was treated with morinidazole sodium chloride injection only , while the observation group received hyperbaric oxygen therapy additionally. Both groups were treated continuously for 3 weeks , and the levels of inflammatory markers , anaerobic bacteria clearance rate , clinical total effect rate , and incidence of adverse reactions were compared before and after treatment , as well as between the two groups. Results:There were no statistically significant difference in inflammatory markers[white blood cell count(WBC) , serum C-reactive protein(CRP) , procalcitonin(PCT),and interleukin-6(IL-6)]between the two groups before treatment( P>0.05 ). After 3 weeks of treatment , the levels of inflammatory markers in both groups were significantly decreased than those before treatment( P<0.05 ), and the levels in the observation group were significantly lower than those in the control group( P<0.05 ). The anaerobic bacteria clearance rate after treatment was 95.65% in the observation group and 74.42% in the control group , with a statistically significant difference( P<0.05 ); the clinical total effective rate after treatment was 97.83% in the observation group and 81.40% in the control group , with a statistically significant difference( P<0.05 ); the incidence of adverse reactions was 8.70% in the observation group and 6.98% in the control group , with no statistically significant difference( P>0.05 ). Conclusion:Morinidazole combined with hyperbaric oxygen therapy is effective in the treatment of Wagner grades 2–4 diabetic foot ulcers , which can more effectively control the infection of diabetic foot ulcers and improve the healing of diabetic foot , compared with using morinidazole alone.
