高压氧联合重复经颅磁刺激对不完全性颈髓损伤术后脊髓功能恢复的影响
Effect of hyperbaric oxygen combined with repetitive transcranial magnetic stimulation on postoperative spinal cord functional recovery in patients with incomplete cervical spinal cord injury
摘要目的:探讨高压氧(HBO)结合重复经颅磁刺激(rTMS)对不完全性颈髓损伤患者手术后脊髓功能恢复的影响。方法:便利选取2019年6月至2022年12月在宁波市第二医院就诊的不完全性颈椎创伤性脊髓损伤患者80例,采用随机数字表法将入选患者分为对照组(rTMS治疗)和观察组(HBO联合rTMS),每组40例,比较2组患者治疗前后的运动评分(MS)、感觉评分(SIS)、改良Barthel指数(MBI)及神经电生理指标变化。结果:治疗1个月后,观察组MS[(45.3±4.8)分]、SIS[(129.7±6.8)分]、MBI[(59.7±3.5)分]均高于对照组[(43.2±4.7)分、(115.8±6.7)分、(45.8±3.7)],且之后各个时间点(治疗1个月、3个月、6个月,随访1年、2年、3年)的上述指标均高于同期对照组,差异均有统计学意义( P<0.05或 P<0.01)。治疗1、3个月后,2组患者的运动诱发电位潜伏期和体感诱发电位N20波幅均高于治疗前,且观察组改善程度优于对照组,差异均有统计学意义(均 P<0.01)。 结论:采用HBO联合rTMS治疗不完全性颈髓损伤有助于促进患者运动和感觉功能的恢复,改善神经电生理指标,提高患者的生活自理能力。

The changes of American Spinal Injury Association(ASIA)motor score(MS),sensory score(SIS),modified Barthel index(MBI),and neuroelectrophysiological indicators were compared between the two groups. Results:After one month of treatment,the observation group showed higher MS[(45.3 ± 4.8)points],SIS[(129.7 ± 6.8)points],and MBI[(59.7 ± 3.5)points]than the control group[(43.2 ± 4.7)points,(115.8 ± 6.7)points,(45.8 ± 3.7)points],with these differences persisting at subsequent corresponding time points(after 1,3,and 6 months of treatments,and follow-up at 1,2,and 3 years after treatment)(all P<0.05 or P<0.01). After 1 and 3 months of treatment,motor evoked potential(MEP)latency and somatosensory evoked potential(SEP)-N20 amplitude improved in both groups compared with baseline,with greater improvement in the observation group(all P<0.01). Conclusion:HBO combined with rTMS promotes motor and sensory functional recovery,improves neuroelectrophysiological indicators,and enhances activities of daily living in patients with incomplete cervical spinal cord injury.
