抗高血压药物相关基因检测在民航飞行员高血压治疗中的初步应用
Initial application of antihypertensive-related gene polymorphism detection in the civil pilots with essential hypertension
摘要目的 探讨高血压药物基因组学在治疗民航飞行员原发性高血压中应用的意义及前景. 方法 对民航某航空公司17例原发性高血压飞行员的3类药物的5个相关基因位点多态性进行检测,以此为指导针对不同患者选择个体化的抗高血压药物进行治疗,并复习高血压药物相关基因近年的研究进展. 结果 部分高血压药物相关位点的多态性预测了个体间药物降压疗效的差异.以药物相关基因检测报告为依据,针对14例原发性高血压飞行员选择了个体化的降压药物,地面观察期间未见不良反应,3周后血压控制均达标,顺利复飞.对于3例临床经验用药后出现药物不良反应和血压控制不理想的患者,以检测报告为指导,对其原有的治疗方案加以调整后,原有不良反应消失、血压控制达标. 结论 所检测的3类药物的5个相关基因的多态性,在民航原发性高血压飞行员的降压药物选择中显示出了良好的指导作用,可提高药物治疗效率,降低药物的不良反应发生率.以基因组为导向的个体化用药模式为今后的发展方向.

abstractsObjective To evaluate the application value and to foresee the prospect of antihypertensive-related gene polymorphism detection in civil pilots.Methods Five gene polymorphisms of antihypertensive corresponding to 3 drug sorts were detected in 17 civil pilots who were diagnosed with essential hypertension (EH).The EH pilots were individually medicated by antihypertensive according to the gene detection tests.The related literatures of antihypertensive pharmacotherapy research progress were reviewed.Results Some antihypertensive-related polymorphism predicted the therapeutic effects of different antihypertensive to individuals.The blood pressure of 14 EH pilots was well controlled by the individual antihypertensive without any side effect and returned to normal range by 3-week medication.They recovered to fly then.Other 3 EH pilots,whose blood pressure was not effectively controlled either by routine medication or due to side effect,regained normal blood pressure with side affect ceasing when they were treated by the drugs in accordance with the gene test.Conclusion The application of 5 antihypertensive-related gene polymorphisms detection for 3 anti-hypertension drugs can provide effective therapy and low adverse drug reaction to EH civil pilots.Drug selection will be genomics-oriented in future.
