摘要目的 探讨下颌阻生牙拔除后两种镇痛方法的效果.方法 将62例患者分为两组,试验组(32例)拔牙后用甲哌卡因局部封闭,对照组(30例)术后2h服用布洛芬缓释胶囊0.3g,此后每隔6h口服一次,至术后48 h结束.比较两种方法术后镇痛效果.结果 试验组术后2h平均疼痛评分(2.5±1.1)低于对照组(2.8±1.1),但两组患者术后2～48 h疼痛程度差异无统计学意义(P＞0.05),疼痛评分与年龄、性别、分组均无关,仅与术后时间有关(P＜0.05).结论 下颌阻生牙拔除术后局部封闭可取得与服用布洛芬缓释胶囊相同的镇痛效果.

abstractsObjective The study is aimed to compare the analgesic effect of two preemptive analgesia programs (partial closure / fenbid po.) in patients undergoing extraction of the impacted mandibular third molar.Methods Sixty-two patients who visited our outpatient center from December 2009 to September 2010 with impacted mandibular third molar were randomly divided into two groups.Patients after extraction were given 3％ mepivacaine injecting immediately under the periosteum for partial closure in the experimental group,while them in the control group,fenbid 300 mg were administrated at 2 hours after operation and once more every 6 hours.Pain sores at 2 h,4 h,8 h,12 h,24 h,48 h after operation were recorded with VAS (visual analogue scale) and parallel groups repeated measures design of longitudinal studies was carried out for statistical comparison between the two groups:studies in which individuals were measured repeatedly through after surgical operation time (2 h,4 h,… ).Results At the time of 2 hours after the operation,the average pain scores of the mepivacaine group (2.5 ± 1.1 ) was lower than the pain scores of the fenbid group (2.8 ± 1.1 ).But there was no significant difference in pain scores between two groups within 2 - 48 h after operation ( P ＞ 0.05 ).Also no statistically relationship between the pain scores and the factors such as age,sex and groups.About the time after operation,between 48 h and 12 h,8 h,4 h,2 h,the pain scores were significantly different ( P ＜ 0.05 ).Conclusions The study suggests that 3％ mepivacaine partial closure immediately after extraction of the impacted mandibular third molar chould have the same analgesic effect as the fenbid.
