口腔红酵母属的分离鉴定、携带状况及药物敏感性研究

摘要目的 分析口腔黏膜病初诊患者唾液真菌培养标本中红酵母属的检出率、病原学及分离人群特征,探讨其在口腔中的可能作用.方法 收集2010年1月至2011年12月间北京大学口腔医学院·口腔医院中医黏膜科唾液真菌培养阳性病例,分析红酵母菌的检出状况.30株菌落数> 200 CFU/ml的红酵母菌经API 20C AUX系统和内转录间隔区基因测序鉴定到种,行体外药敏试验,并回顾性分析分离人群的临床特征.结果 2010年和201 1年红酵母属的检出率(占全年唾液培养阳性标本的百分比)分别为2.5%(78/3113)和2.9%(82/2823);30株红酵母菌中2株为黏红酵母菌,28株为胶红酵母菌,对唑类药物抑制90%受试菌时的最小抑菌质量浓度为氟康唑>128 mg/L;伊曲康唑>4 mg/L;伏立康唑=4 mg/L.携带红酵母属的病例以口干[30%(9/30)]和高血糖患者[30%(9/30)]居多.结论 红酵母属可分离自口腔黏膜病患者唾液,对唑类药物敏感性较差,其携带可能与口腔及全身状况相关.

abstractsObjective To explore the possible role of oral carriage of Rhodotorula spp (Rho) by preliminarily investigating the detection rate in oral mucosal disease patients,microbiological characteristics,population features and in vitro antifungal drug susceptibility.Methods The positive saliva fungi culture cases were collected during Jan 2010 to Dec 2011 and the detection ratio of Rho from the positive fungal culture was analyzed.Thirty strains with CFU > 200 per ml were collected and identified.The conventional API 20 C AUX kit supplemented by sequencing the ITS region of each strain was used in the identification of each strain.Additionally,the in vitro antifungal susceptibility tests were performed by using ATB FUNGUS 3 kit according to CLSI M27-A2 document.Clinical data of 30 cases carrying the isolated stains were analyzed retrospectively.Results The Rho ratio in 2010 and 2011 was 2.5%(78/3113)and 2.9%(82/2823)respectively.Twenty-eight strains were identified as Rhodotorula mucilaginosa,and the other 2 strains were identified as Rhodotorula glutinis.The result of in vitro antifungal susceptibility test of Rho revealed rather high MIC90 value to three azole drugs (fluconazole > 128 ; itraconazole > 4 ; voriconazole =4).Nine of the 30 cases(30%) were with Sjogren syndrome/dry mouth conditions,and nine of the 30 cases(30%) were with diabetes mellitus/hyperglycemia.Conclusions Oral Rho can be isolated from patients with oral mucosal diseases.The drug susceptibility to azoles was low.Oral carriage of Rho might be associated with oral diseases and systemic disorders.
