摘要选择165例2型糖尿病患者,其中合并胫前色素沉着斑(PPP)者80例(PPP 组),无 PPP 者85例(NPPP 组),以及50名糖调节正常者为对照组(NGR 组)。记录并比较各组患者的性别、年龄、糖尿病病程、体重指数(BMI)、收缩压、舒张压、空腹血糖(FBG)、餐后2 h 血糖(2hPG)、甘油三酯(TG)、总胆固醇、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、HbA1C、视黄醇结合蛋白4(RBP4)、胱抑素 C 等。结果显示, NPPP 组 BMI、FBG、2hPG、TG、胱抑素 C 高于 NGR 组(P<0.01)。 PPP 组患者 BMI、收缩压、FBG、2hPG、TG、胱抑素 C、RBP4高于 NGR 组(P<0.05或 P<0.01)。 PPP 组患者糖尿病病程、FBG、2hPG、HbA1C、胱抑素 C、RBP4明显高于 NPPP 组(P<0.01)。 Pearson 相关分析显示血清 RBP4与胱抑素 C 呈线性正相关(r =0.77, P<0.01)。受试者工作特征曲线显示 RBP4及胱抑素 C 曲线下面积分别为0.81、0.78(P<0.01)。二元logistic 回归分析结果显示,糖尿病病程、HbA1C、RBP4与 PPP 的发生呈正相关(r =0.37、0.26、0.22,P<0.05或 P<0.01)。

abstracts[Summary] A total of 165 type 2 diabetic patients were divided into two groups with pigmented pretibial pathes(PPP group) and no PPP( NPPP group). 50 subjects with normal glucose regulation were used as a control group(NGR group). The records of sex, age, diabetes duration, body mass index( BMI), systolic blood pressure (SBP), diastolic blood pressure, high density lipoprotein-cholesterol, low density lipoprotein-cholesterol, fasting blood glucose(FBG), 2 h postpradial plasma glucose(2hPG), triglyceride(TG), total cholesterol, HbA1C , retinol binding protein 4(RBP4), cystatin C(Cys C)were analyzed. The results showed that BMI,FBG, 2hPG, TG, and Cys C levels in NPPP group were higher than those in NGR group(all P<0. 01). The levels of BMI, SBP, FBG, 2hPG, TG, Cys C, and RBP4 in PPP group were higher than those in NGR group(P<0. 01 or P<0. 05), while diabetes duration, FBG, 2hPG, HbA1C , Cys C, and RBP4 in PPP group were higher than those in NPPP group ( P <0. 01). Pearson correlation analysis revealed that serum RBP4 and Cys C were in linear positive correlation(r=0. 77, P< 0. 01). The areas under receiver operating characteristic curve of RBP4 and Cys C were 0. 81 and 0. 78, respectively(P<0. 01). The results of binary logistic regression analysis showed that diabetes duration, HbA1C , RBP4 were related to PPP(r=0. 37, 0. 26, 0. 22, P<0. 05 or P<0. 01).
