摘要目的 探讨液泡膜-ATP酶(Vacuolar-H+-ATPase,V-ATPase)与P-糖蛋白(P-glyeoprotein,P-gp)在结肠癌组织中的表达及其临床意义.方法 应用免疫组织化学方法检测80例结肠癌组织、20例结肠腺瘤组织和10例正常结肠黏膜组织中V-ATPase与P-gp蛋白的表达,分析它们的相关性及其与结肠癌患者的临床病理特征和预后的关系.结果 V-ATPase和P-gp蛋白在结肠癌组织中阳性表达率(72％和80％)高于结肠腺瘤组阳性表达率(40％和35％)及正常结肠黏膜组阳性表达率(20％和20％),差异均有统计学意义(均P＜0.05).结肠癌组织中V-ATPase蛋白表达与P-gp蛋白表达呈正相关(r=0.567,P＜0.01),V-ATPase蛋白和P-gp蛋白的表达与肿瘤的TNM分期、淋巴结转移和肿瘤分化程度有关(均P＜0.05).V-ATPase蛋白高表达结肠癌患者的5年生存率低于低表达者(P=0.023),5年复发率高于低表达者(P=0.024).结论 V-ATPase在结肠癌组织中表达上调,与结肠癌的侵袭和转移有关,V-ATPase蛋白高表达患者的预后不良.

abstractsObjective To investigate the expression of Vacuolar-H +-ATPase (V-ATPase) and P-glyeoprotein (P-gp) protein in colon carcinoma tissues,the correlation between the expression of V-ATPase and P-gp and their clinicopathological significance.Methods In samples from 80 cases of colon cancer,20 cases of colon adenoma and 10 cases of normal colonic mucosa tissues,the expression of V-ATPase and P-gp protein were detected by immunohistochemical method,their relationship was analyzed,the clinicopathological features and prognosis were evaluated.Results In colon cancer,V-ATPase and P-gp protein expression was 72％ and 80％,higher than that in colon adenomas (40％,35％),and in normal colon mucosa (20％,20％),the difference was statistically significant (P ＜ 0.05).There was a positive correlation between the expression of V-ATPase and that of P-gp (r =0.567,P ＜0.01).V-ATPase and P-gp protein expression in colon cancer was associated with TNM stage,lymph node metastasis and tumor differentiation (P ＜ 0.05).Patients with high V-ATPase expression had lower 5-year survival rate than those with low V-ATPase expression (P =0.023),and 5-year recurrence rate was higher than those with low expression (P =0.024).Conclusions The expression of V-ATPase is up-regulated in colon cancer,there is a positive correlation with colon cancer progress and metastasis,and high V-ATPase protein expression predicts poor prognosis.
