摘要目的 探讨神经内镜治疗室间孔区病变的适应证及其应用价值.方法 应用神经内镜治疗20例室间孔区病变.实质性肿瘤烧灼基底后,使肿瘤逐渐脱离室间孔,用活检钳将肿瘤钳住,连同内镜一并退出.囊性病变根据性质采用抽吸囊液后切除囊壁或直接切除囊壁.单纯室间孔闭寨者,埘粘连物较薄的行穿通术.结果 9例星形细胞瘤全切除,1例原始神经外胚叶瘤部分切除;囊性病变囊壁全切除2例,部分切除4例.2例室间孔粘连物较薄者被穿通,2例较厚者行透明隔造瘘.全部病例无明显并发症,远期效果良好.结论 神经内镜治疗室间孔区病变方法 可行,安全有效,能明显减少对脑组织的切开、牵拉和重要结构的损伤.

abstractsObjective To probe the indications and applied value of endoscopic treatment of the lesions in foramen of Monro region. Method20 cases of lesions in foramen of Monro region were selected. The bases of the solid tumors were cauterized,which made the tumors away from the foramen of Monro gradually,and the tumors were hold and took by forceps with endoscope. The cyst walls were excised directly or after cyst liquid taken out according to the quality of the lesions. The thinner obstructions of the foramen of Monro were penetrated. Results 9 astrocytomas were totally resected, 1 primitive neuroectodermal tumor was partially resected ;The walls of the cyst lesions were totally removed in 2 cases and partially excised in 4 ones. 2 cases of thin obstruction of the foramen of Monro were penetrated. All patients had no obvious complications, and had good long-term follow-up results. Condnsions Neuroendoscopic technique is feasible in the lesions in foramen of Monro region, which can improve the safety and the efficiency, and minimize brain trauma.
