计算机三维重建技术下模拟侧脑室额角穿刺方法的探讨

摘要目的 利用计算机三维重建技术探讨侧脑室中度扩大患者的额角穿刺方法.方法 随机选取本院70例成人头颅三维螺旋CT的原始资料,从中筛选42例作为研究对象,利用三维重建技术建立颅骨-侧脑室三维虚拟模型,并采用二种不同侧脑室额角穿刺方法进行模拟穿刺,方法1:选择传统穿刺部位和方向,即取冠状缝中点前2 cm,中线旁开2.5 cm作为穿刺点,穿刺方向平行于矢状面,向外耳道连线方向穿刺;方法2:选择传统穿刺部位,但穿刺方向朝向外耳道假想连线中点.结果 42例模拟穿刺中方法1和方法2分别穿刺成功25例、39例,统计学结果显示方法2穿刺成功率明显高于方法1(P =0.0003<0.05).结论 对于侧脑室中度扩大患者,行额角穿刺方法2优于方法1.

abstractsObjective To explore the lateral ventricle puncture methods for patients with moderate expansion using computer three-dimensional reconstruction technique.Methods The CT scan data of 42 cases out of 70 adult patients were selected for this study.The three-dimensional reconstruction techniques were used to establish the skull-lateral ventricle virtual model,and two kinds of different methods were used to simulate the lateral ventricle puncture (Including method one:traditional puncture site and direction:select the position where 2cm in front of the middle point of coronal suture and incise 2.5cm along with the middle line as the puncture position,make the puncture direction parallel to the vertical plane and along with the line of the external auditory canal; method two:puncture direction toward the midpoint of the external auditory canal,the other same to traditional methods).Results The two puncture methods were successful for 42 cases in 25 cases,39 cases,respectively.The statistical results showed that method two had a significantly higher successful rate compared to that of method one.(P =0.0003) Conclusions For lateral ventricle puncture in patients with moderate expansion of ventricles,method two was better than method one.
