额叶胶质瘤患者额顶网络的静息态功能磁共振研究
A resting-state functional MRI study on fronto-parietal network in patients with unilateral frontal lobe glioma
摘要目的:初步探讨额叶胶质瘤患者静息状态下额顶网络内功能连接强度变化与肿瘤级别的关系。方法:回顾性分析2020年1—12月首都医科大学附属北京天坛医院神经外科行手术治疗的44例额叶胶质瘤患者的临床资料,其中高级别胶质瘤(HGG)25例,低级别胶质瘤(LGG)19例;同时纳入与患者性别和年龄相匹配的21名健康对照者。全级别胶质瘤组和健康对照组均完成静息态功能磁共振扫描,以额顶网络内4个重要脑区为感兴趣区,分别为左外侧前额叶皮质(LPFC_L)、右外侧前额叶皮质(LPFC_R)、左后顶叶皮质(PPC_L)和右后顶叶皮质(PPC_R),经过数据处理得到各脑区间的功能连接强度值,并进行组间比较和Spearman相关分析。结果:与健康对照组相比,全级别胶质瘤组的LPFC_L与PPC_L以及LPFC_R与PPC_L的功能连接强度值均显著减小(均 P<0.05)。HGG组、LGG组和健康对照组的LPFC_L与PPC_L以及LPFC_R与PPC_L的功能连接强度值的差异均有统计学意义(均 P<0.05);与健康对照组相比,HGG组的LPFC_L与PPC_L以及LPFC_R与PPC_L的功能连接强度值均显著减小(均 P<0.05),而LGG组各脑区间功能连接强度值的差异均无统计学意义(均 P>0.05)。Spearman相关分析显示,肿瘤体积与各脑区间的功能连接强度值均无相关性(均 P>0.05)。全级别胶质瘤组的功能连接强度值的相关系数显著大于健康对照组( P=0.009)。HGG组、LGG组和健康对照组的功能连接强度值的相关系数比较差异有统计学意义( P=0.036);与健康对照组比较,功能连接强度值的相关系数在HGG组显著增大( P=0.044),在LGG组的差异无统计学意义( P=0.098)。 结论:额叶胶质瘤患者静息状态下额顶网络内功能连接强度减弱,高、低级别脑胶质瘤患者额顶网络发生的功能改变不同,高级别脑胶质瘤患者的额顶网络功能连接强度显著减弱。

abstractsObjective:To preliminarily explore the relationship between the changes of functional connectivity in the fronto-parietal network (FPN) and tumor grade in patients with frontal gliomas in the resting state.Methods:A retrospective analysis was conducted on the clinical data of 44 patients with frontal glioma who underwent neurosurgery at Department of Neurosurgery, Beijing Tiantan Hospital, Capital Medical University from January to December 2020. Those patients included 25 cases of high-grade glioma (HGG) and 19 cases of low-grade glioma (LGG). Twenty-one healthy controls (HC) matched with the patient′s sex and age were also included. All patients and healthy controls completed resting-state fMRI scans. Four important brain regions in the FPN were used as regions of interest, namely the left lateral prefrontal cortex (LPFC_L) and the right lateral prefrontal cortex (LPFC_R), left posterior parietal cortex (PPC_L) and right posterior parietal cortex (PPC_R). After data processing, the strength values of functional connectivity between each brain regions were obtained, and the comparison between groups and Spearman correlation analysis were carried out.Results:Compared with the HC group, the patients with frontal lobe glioma showed decreased functional connectivity between the LPFC_L and the PPC_L, as well as between the LPFC_R and the PPC_L (all P<0.05). There were statistically significant differences in the functional connectivity between LPFC_L and PPC_L, and between LPFC_R and PPC_L among the HGG, LGG and HC groups (all P<0.05). Compared with the HC group, the HGG group showed decreased functional connectivity between the LPFC_L and the PPC_L, as well as between the LPFC_R and the PPC_L (both P<0.05). However, there was no significant difference in the functional connectivity between those areas in the LGG group (both P>0.05). Spearman correlation analysis showed that there was no correlation between tumor volume and functional connectivity between the brain regions (all P>0.05). In addition, the correlation coefficient of functional connectivity strength values was higher in patients with frontal lobe glioma than in the HC group ( P=0.009). There was a statistically significant difference in the correlation coefficient among the HGG, LGG and HC groups ( P=0.036). The correlation coefficient of functional connectivity strength values was higher in the HGG group than in the HC group ( P=0.044), whereas no statistical difference was found between the LGG group and the HC group ( P=0.098). Conclusions:The resting-state functional connectivity of the FPN is decreased in patients with frontal lobe gliomas. The functional changes of the FPN are different in patients with high-grade and low-grade gliomas. The functional connectivity strength of the FPN in patients with high-grade gliomas is significantly reduced.
