摘要目的 评价动力性外固定器治疗指骨关节内骨折、脱位的临床效果.方法 应用动力性外固定器结合内固定治疗指骨关节内骨折、脱位16例.结果 术后平均随访时间为13.4周,14例开放伤口全部愈合.16例中有3例出现针道感染,通过加强局部护理得到改善;除1例因重度疼痛无法忍受外,其余患者均认为可以耐受该外固定支架,基本不影响日常生活,术后伤指无伸肌腱粘连.随访X线片见骨折复位效果满意,关节脱位得到纠正,关节间隙恢复正常.结论 动力性外固定器价格低廉、操作简单、损伤小,能够对关节进行有效固定,维持关节牵引,允许关节早期功能锻炼,是一种较理想的治疗指骨关节内骨折、脱位的方法.

abstractsObjective To evaluate the treatment outcomes of dynamic external fixation device in the management of intraarticular phalangeal fracture.dislocation of the hand. Methods Sixteen cases of intraarticular Dhalangeal fracture-dislocation were treated by dynamiC external fixator combined with limited internal fixation. Results The average postoperative follow-up was 13.4 weeks.Open wounds healed uneventfully in all 14 cases.Pin tract infection was seen in 3 cases and was resolved by wound care.All but one patient tolcrated the external fixator well and reported that it did not interfere with activities of daily living.There was no obvious postoperative extensor tendon adhesion.X-rays showed satisfactory reduction of the fracture and joint dislocation.The normal joint space was restored. Conclusion Dynamic external fixator is a cost effective device.The procedure is simple and causes minor tissue injury.It cart obtain reliable fixation,maintain the traction and permit early mobilization of the joint.
