摘要目的:探讨腕关节轴向骨折脱位的治疗效果。方法:自2013年8月至2018年5月，我们共收治8例腕关节轴向骨折脱位患者。根据Garcia-Elias分型：尺侧轴向骨折脱位4例(均为开放性损伤)，桡侧轴向骨折脱位2例(均为开放性损伤)，桡尺侧联合轴向骨折脱位2例(1例开放性损伤、1例闭合性损伤)。均行急诊手术，闭合性损伤行骨筋膜室切开减张、神经减压、复位并固定骨折与脱位，开放性损伤患者行清创复位固定骨折与脱位，修复肌腱、神经、血管；2例患者二期行植皮术修复软组织缺损。早期开始康复治疗。结果:术后8例患者随访4～15个月，平均9个月。根据中华医学会手外科学会上肢部分功能评定试用标准评定疗效：优1例，良3例，可3例，差1例。结论:腕关节轴向骨折脱位的软组织损伤程度远大于骨与关节损伤程度，应重视软组织损伤的处理及术后系统康复治疗。

abstractsObjective:To investigate the clinical efficacy of axial fracture dislocation of wrist joint.Methods:From August 2013 to May 2018, 8 patients with axial fracture and dislocation of wrist joint were treated. According to the Garcia-Elias classification, there were 4 cases of ulnar axial fracture and dislocation (all open injury), 2 cases of radial axial fracture and dislocation (all open injury), and 2 cases of combined radial and ulnar axial fracture and dislocation (1 case of open injury and 1 case of closed injury). All the patients underwent emergency surgery, including open reduction of bone fascial compartment, nerve decompression, reduction and fixation of fractures and dislocations; patients with open injury underwent debridement and reduction to fix fractures and dislocations, and repair tendons, nerves and blood vessels. Two patients underwent skin grafting to repair soft tissue defects. The rehabilitation treatment was started at early stage.Results:Eight patients were follow-up for 4 to 15 months with an average of 9 months. According to the evaluation standard of upper limb function of Hand Surgery Society of Chinese Medical Association, the results were rated as excellent in 1 case, good in 3 cases, fair in 3 cases and poor in 1 case.Conclusion:The degree of soft tissue injury of axial fracture dislocation of wrist joint is much greater than that of bone and joint injury. The attention should be paid to the treatment of soft tissue injury and postoperative rehabilitation treatment.
