abstractsObjective:To investigate the clinical efficacy of microhook plates for the treatment of old bony mallet fingers. Methods:A retrospective analysis was conducted on 18 patients with old bony mallet fingers admitted to our department from May 2021 to May 2024. The treatment plan was open reduction and microhook plate fixation, and postoperative braces were fixed for 4 to 6 weeks. Results:All 18 patients were follow-up after surgery for a period of 6 to 33 months, with an average of 18 months. At the last follow-up, according to the Crawford functional evaluation criteria, the finger injury function was assessed as excellent in 10 cases, good in 4 cases, fair in 3 cases, and poor in 1 case. The average degree of extension of the distal interphalangeal (DIP) joint in the affected finger was measured to be 22° (5° to 50°) before surgery. After surgery, the degree of extension of the DIP joint improved to an average of 3.5° (0° to 15°), and the average degree of active flexion of the DIP joint was 55° (30° to 80°). Four patients experienced flexion restriction after surgery. All patients did not experience complications such as incision infection or skin necrosis. One patient developed deck deformity, and after 3 months of fracture healing and internal fixation removal, the appearance of the deck was restored. Conclusion:Microhook plate therapy has significant clinical efficacy in treating old bony mallet fingers, with few complications, and is worthy of clinical promotion.
