摘要目的:探讨抗生素骨水泥联合股前外侧游离皮瓣修复手部热压伤的临床治疗可行性及疗效。方法:自2019年1月至2022年12月我们共收治手部热压伤患者12例，急诊一期均行坏死组织彻底清创、深部组织损伤探查修复、创面缺损处行含庆大霉素的抗生素骨水泥覆盖，术后3~7 d早期选择游离股前外侧皮瓣修复创面，并随访治疗效果。结果:本组12例均顺利完成手术，创面未出现感染并发症，12例皮瓣完全存活，创面均Ⅰ期愈合，皮瓣质地、外观与周围正常皮肤相似。术后随访时间为6~14个月，平均8.2个月，手功能恢复情况根据中华医学会手外科学会上肢部分功能评定试用标准评价：优7例，良4例，可1例。结论:抗生素骨水泥联合股前外侧游离皮瓣修复手部热压伤创面，可有效防治感染，改善患者预后，值得临床推广应用。

abstractsObjective:To investigate the clinical feasibility and efficacy of antibiotic-laden bone cement combined with anterolateral femoral free flap in the repair of hand thermal compression injury.Methods:From January 2019 to December 2022, 12 cases of patients with hand thermal compression injury admitted to our hospital were selected. In the first stage of the emergency department, the necrotic tissue was completely debrided, deep tissue injury was explored and repaired, and the wound defect was covered with antibiotic-laden bone cement containing gentamicin. The anterolateral femoral free flap was selected for wound repair 3 to 7 days after surgery and treatment outcomes were followed up.Results:All 12 cases successfully completed the surgery without any infection or complications. The skin flaps in all 12 cases survived completely, and the wounds achieved primary healing. The texture and appearance of the skin flaps were similar to the surrounding normal skin. The postoperative follow-up period was 6 to 14 months, with an average of 8.2 months. According to the evaluation criteria for upper limb functional assessment of the Chinese Medical Association Hand Surgery Society, the recovery of hand function was evaluated as excellent in 7 cases, good in 4 cases, and fair in 1 case.Conclusion:The combination of antibiotic-laden bone cement and anterolateral femoral free flap for repairing hand thermal compression wounds can effectively prevent and treat infections, improve patient prognosis, and is worthy of clinical promotion and application.
