摘要目的 探讨血清玻连蛋白(VTN)与前列腺特异性抗原(PSA)联合检测前列腺癌的意义.方法 对30例前列腺癌转移(PCaM)患者、32例前列腺癌未转移(PCa)患者及41例良性前列腺增生(BPH)患者以及实行前列腺癌根治术的患者术前和术后血清中VTN、PSA的浓度进行分析.结果BPH组、PCa组和PCaM组血清VTN的浓度分别为(219.63 ±25.30)、(201.72±19.37)、(170.05 ±23.80) μg/L,差异有统计学意义(P ＜0.05)；PCa组术前、术后血清VTN的浓度分别为(204.22±20.57)、(217.56±11.32) μg/L,差异有统计学意义(P＜0.01)；PCa组血清PSA、VTN以及联合检测的Youden指数分别为0.3468、0.2620和0.5635；Kappa值分别为0.338、0.304和0.448;受试者工作特征(ROC)曲线下面积(AUC)分别为0.625、0.673和0.703,差异均有统计学意义(P＜0.05).结论 BPH组血清VTN的浓度高于PCa组,PCa组又高于PCaM组；PCa组患者术前血清VTN的浓度低于术后；血清PSA和VTN联合诊断前列腺癌优于单项诊断.

abstractsObjective To evaluate the significance of the combined assay of Vitronectin (VTN) and prostate specific antigen (PSA) in the diagnosis of prostate cancer.Methods Serum VTN and PSA levels were detected in 30 cases of prostate cancer with metastasis (PCaM),32 cases of prostate cancer patients without metastasis (PCa),41 cases of benign prostatic hyperplasia (BPH) before and after operation.Results The levels of VTN in BPH group,PCa group and PeaM group were (219.63 ± 25.30),(201.72 ± 19.37) and (170.05 ± 23.80) μg/L respectively,with the difference being statistically significant (P ＜ 0.05).The levels of VTN in PCa pre-operation group and post-operation group were (204.22 ±20.57) and (217.56-11.32) μg/L respectively,with the difference being statistically significant (P ＜0.01).In the PCa group,Youden indexes of PSA,VTN and combined detection were 0.3468,0.2620 and 0.5635 ; Kappa values were 0.338,0.304 and 0.448 ; the areas under receiver operating characteristic (ROC) curve were 0.625,0.673 and 0.703 respectively,with all differences being statistically significant (P ＜ 0.05).Conclusion VTN was more highly expressed in the serum of BPH patients than PCa patients,and was also more highly expressed in the serum of PCa patients than PCaM patients.The combined assay of serum VTN and PSA is of significantly clinical value in raising the diagnosis rate of prostate cancer.
