摘要目的 探讨血清微小RNA(miRNA,miR)-183在结肠癌诊断中的价值.方法 选取89例结肠癌患者作为病例组,选取50名健康志愿者作为对照组.对两组研究对象的血清miR-183、癌胚抗原(CEA)、糖蛋白抗原-724(CA-724)水平进行检测和比较.结果 病例组患者血清miR-183、CEA、CA-724表达水平显著高于对照组,两组差异均有统计学意义(P=0.001).TNM分期为Ⅲ～Ⅳ期及存在淋巴结转移的结肠癌患者的血清miR-183、CEA、CA-724表达水平高于TNM分期为Ⅰ～Ⅱ期的患者,存在淋巴结转移的结肠癌患者的血清miR-183、CEA、CA-724表达水平高于无淋巴结迁移的患者,差异有统计学意义(P =0.006).在结肠癌的诊断中,血清miR-183水平的受试者工作特征曲线下面积(AUC)最高,为0.958,95％可信区间(CI)为0.931 ～0.985.在早期结肠癌的诊断中,血清miR-183水平的AUC最高,为0.883,95％ CI为0.809 ～0.956.结论 结肠癌患者表现为血清miR-183水平的上调,其水平与肿瘤的侵袭和扩散程度明显相关.

abstractsObjective To study and analysis of the value of serum microRNA (miRNA,miR)-183 in the diagnosis of colon cancer.Methods 89 cases of the patients with colon cancer were selected as the case group,50 healthy volunteers were selected as the control group.The serum expressions of miR-183,the carcinoembryonic antigen (CEA),the carbohydrate antigen-724 (CA-724) of the subjects in the two groups were detected and compared.Results The serum expressions of miR-183,CEA,CA-724 of the patients in the case group were significantly higher than those in the control group,the differences between the two groups were statistically significant (P =0.001).The serum expressions of miR-183,CEA,CA-724 of the patients at TNM Ⅲ-Ⅳ stage were significantly higher than those at TNM Ⅰ-Ⅱ stage.The serum expressions of miR-183,CEA,CA-724 of the patients with lymph node metastasis were significantly higher than those without lymph node metastasis.The difference was statistically significant (P =0.006).In the diagnosis of colon cancer,the area under the receiver operating characteristic curve (AUC) of serum miR-183 was the highest (0.958) and the 95％ confidence interval (CI) was (0.931-0.985).In the diagnosis of early colon cancer,AUC of serum rniR-183 was the highest (0.883) and the 95 ％ CI was (0.809-0.956).Conclusion The patients colon with cancer patients show overexpression of serum miR-183.The level is related with the degrees of invasion and diffusion of the tumor,and can be used as the serum markers.in early diagnosis and evaluation of disease of colon cancer.
