摘要目的:观察全身振动训练对脑卒中偏瘫患者平衡功能及步行效率的作用。方法:将符合入组条件的30例脑卒中患者使用随机数字表法分为对照组( n＝15)与实验组( n＝15)。对照组接受常规脑卒中康复治疗(每日2次，每次30 min，每周6 d，连续训练2周)和假性全身振动治疗(每日1次，每次共14 min，每周6 d，连续治疗2周)；实验组在接受常规脑卒中康复治疗的基础上，接受全身振动训练(振动训练频率10 Hz，振幅4 mm，每日1次，每次共14 min，每周6 d，连续治疗2周)。分别于治疗前和治疗2周后(治疗后)对2组受试者进行平衡功能评估(前后和左右偏移能力、患侧单腿站立时间)和步行效率评估(10 m步行试验、3 m坐站试验)。 结果:治疗后，2组患者前后偏移能力和左右偏移能力、患侧单腿站立时间、10 m步行试验结果、3 m坐站试验结果较组内治疗前均显著改善，差异均有统计学意义( P<0.05)。实验组治疗前、后改善幅度即左右偏移能力差值为(0.94±0.56)°，患侧单腿站立时间差值为(2.51±1.31)s，10 m步行时间差值为(－2.76±1.04)s，3 m坐站试验差值为(－3.58±1.90)s，与对照组比较，差异均有统计学意义( P<0.05)，而前后偏移能力差值与对照组比较，差异无统计学意义( P>0.05)。 结论:全身振动训练可有效地改善脑卒中偏瘫患者的平衡功能和步行效率，且治疗效果较常规康复治疗更佳。

abstractsObjective:To investigate the effect of whole body vibration (WBV) on balance and walking ability after a stroke.Methods:Thirty stroke survivors were randomly divided into a control group ( n=15) and an experimental group ( n=15). Both groups received 30 minutes of routine rehabilitation training twice a day, 6 days a week for 2 weeks. The experimental group was additionally provided with 14 minutes of 10Hz WBV with an amplitude of 4mm every day during the 2 weeks. Before and after the intervention, balance was quantified using the front-behind inclination angle (F-BIA), the left-right inclination angle (L-RIA) and the single-leg stance time of the hemiplegic side (SSTHS). Walking efficiency was quantified using the 10 metre walk test (10MWT) and the 3 metre timed up-and-go test (3TUG). Results:There was a significant improvement in all of the measurements in both groups after the intervention, but the improvements in average L-RIA, SSTHS, 10 MWT and 3TUG results in the experimental group were significantly greater than those in the control group.Conclusions:Supplementing routine therapy with WBV can improve balance and walking ability after a stroke more effectively than routine therapy alone.
