"六字诀"训练联合肺康复训练对肺结节切除术后患者功能恢复的影响
The effects of combining Liuzijue training with pulmonary rehabilitation on functional recovery after pulmonary nodule resection
摘要目的:观察"六字诀"训练联合肺康复训练对肺结节切除术后患者功能恢复的影响。方法:采用随机数字表法将60例肺结节切除术后患者分为观察组及对照组,每组30例。对照组术后给予肺康复训练,观察组在此基础上辅以"六字诀"训练,共训练4周。于手术前、术后4周时及3个月后随访时对2组患者进行疗效评定,采用肺功能仪检测2组患者肺功能,具体检测指标包括用力肺活量(FVC)、第1秒用力呼气容积(FEV1),分别采用6 min步行试验(6MWT)及简易体能状况量表(SPPB)评定患者运动耐力和躯体功能情况,采用36条目简明健康调查量表(SF-36)评定患者生活质量情况。结果:术后4周时2组患者肺功能指标、6MWT、SPPB以及SF-36量表各维度评分均明显不及手术前水平( P<0.05);术后3个月随访时2组患者上述指标均较术后4周时有不同程度改善;并且术后4周时观察组FVC[2.10±0.52(L)]、FEV1[1.59±0.43(L)]、SF-36量表躯体疼痛(BP)评分[62.00(52.00,80.00)分]、整体健康(GH)评分[87.00(80.00,87.00)分]、活力状况(VT)评分[95.00(88.75,96.25)分]、社会功能(SF)评分[100.00(88.00,100.00)分]及情感职能(RE)评分[100.00(100.00,100.00)分]均显著优于同期对照组水平( P<0.05),3个月后随访时观察组FVC[2.62±0.57(L)]、FEV1[1.97±0.51(L)]、6MWT[522.06±55.96(m)]、SPPB评分[12.00(12.00,12.00)分]、GH评分[97.00(95.75,97.00)分]、VT评分[100.00(98.75,100.00)分]及SF评分[100.00(100.00,100.00)分]亦显著优于同期对照组水平( P<0.05)。 结论:"六字诀"训练联合肺康复训练可显著提高肺结节切除术后患者的肺功能,进一步改善患者运动耐力、躯体功能及生活质量,该中西医结合治疗方式值得临床推广、应用。

abstractsObjective:To explore any effect of combining Liuzijue training with pulmonary rehabilitation training on functional recovery after pulmonary nodule resection.Methods:Sixty patients after pulmonary nodule resection were randomly divided into an observation group and a control group, each of 30.Both groups were given pulmonary rehabilitation training, but the observation group also received Liuzijue training for 4 weeks. Before and after the treatment and at follow-up 3 months after the surgery, both groups′ forced vital capacity (FVC) and forced expiratory volume in one second (FEV1) were evaluated. Exercise endurance and somatic functioning were quantified using the 6-minute walk test (6MWT) and the short physical performance battery (SPPB), respectively. The 36-item short form survey (SF-36) was administered to assess their life quality.Results:After 4 weeks the pulmonary function index, 6-minute walking distance, SPPB scores and SF-36 scores of both groups were significantly improved. Both groups′ improvements had persisted 2 months later. The observation group′s average FVC and FEV1 values, as well as their average scores on some components of the SF-36 instrument were significantly better than the control group′s averages after 4 weeks. After 3 months their average 6MWT distance and SPPB score was also significantly better.Conclusions:Combining Liuzijue training with pulmonary rehabilitation training can significantly improve the pulmonary function of patients after pulmonary nodule resection. It can further improve their exercise endurance, somatic function and life quality. The combination is worthy of clinical promotion and application.
