山西省鼠伤寒沙门菌耐药与分子分型研究
Drug resistance analysis and molecular typing of Salmonella typhimurium isolates in Shanxi Province
摘要目的:了解山西省食源性疾病主动监测中鼠伤寒沙门菌的耐药谱及分子分型,为鼠伤寒沙门菌的防控、溯源及合理使用抗生素提供科学依据。方法:对2015—2019年山西省食源性疾病主动监测病例中分离到的鼠伤寒沙门菌进行血清学分型、药物敏感性试验以及脉冲场凝胶电泳(PFGE)分子分型。结果:2015—2019年共分离到鼠伤寒沙门菌63株,其中37株抗原式为[4,5,12,:i:-],是鼠伤寒沙门菌主要血清型。63株鼠伤寒沙门菌对14种抗生素有不同程度的耐药性。对亚胺培南全部敏感(100%),对头孢西丁、头孢他啶、头孢噻肟、阿奇霉素、庆大霉素的敏感率大于92.00%,对四环素的耐药率最高(82.54%)。40株为多重耐药株。结论:山西省鼠伤寒沙门菌耐药情况比较严重,仍需要加强对沙门菌多重耐药的监测。而PFGE带型呈现出多样性的同时,又具有显著的优势带型,说明山西省鼠伤寒沙门菌存在高度散发和聚集性暴发的流行特征。

abstractsObjective:To investigate the drug resistance spectrum and molecular types of Salmonella typhimurium strains collected in an active surveillance of foodborne diseases in Shanxi Province in order to provide reference for the prevention, control and tracing of Salmonella typhimurium as well as the rational use of antibiotics. Methods:Clinical Salmonella typhimurium isolates were collected in an active surveillance of food-borne disease in Shanxi Province from 2015 to 2019. Serological typing, antibiotic susceptibility test and pulsed-field gel electrophoresis (PFGE) molecular typing were conducted. Results:A total of 63 Salmonella typhimurium strains were isolated from 2015 to 2019 and 37 of them belonged to the [4, 5, 12, : i: -] serotype, which was the predominant serotype of Salmonella typhimurium. The 63 Salmonella typhimurium strains showed varying degrees of resistance to 14 antibiotics. They were all sensitive to imipenem (100%), and more than 92.00% were sensitive to cefoxitin, ceftazidime, cefotaxime, azithromycin and gentamicin. The resistance rate to tetracycline was the highest (82.54%). Forty strains were multidrug resistant. Conclusions:Drug resistance in Salmonella typhimurium was serious in Shanxi Province and the surveillance of multidrug-resistant Salmonella typhimurium should be strengthened. PFGE banding patterns not only showed diversity, but also the predominant type, which indicated that Salmonella typhimurium strains were characterized by highly sporadic infections and massive outbreaks in Shanxi Province.
