新型介电传感标测系统对比剂注射法在心房颤动冷冻球囊消融术中评估肺静脉封堵情况的可行性与准确性
Feasibility and accuracy of a novel dielectric sensing tool, injection module, for pulmonary vein occlusion during cryoballoon ablation of atrial fibrillation
摘要目的:验证新型介电传感标测系统对比剂注射法在心房颤动患者冷冻球囊消融(CBA)术中评估肺静脉封堵情况的可行性与准确性。方法:前瞻性连续纳入2021年8月至2022年1月于北部战区总医院应用新型介电传感标测系统辅助进行CBA的房颤患者。在应用该系统及8极环形标测导管进行左心房及肺静脉三维成像后,使用该系统封堵评估工具对比剂注射法评估肺静脉封堵情况,以X线透视下造影剂滞留情况为"金标准"进行对比。结果:共入选35例患者,年龄(61±8)岁,男27例(77.1%,27/35),阵发性房颤23例(65.7%,23/35),持续性房颤12例(34.3%,12/35)。共进行148根肺静脉的封堵评估验证。以X线透视为金标准,对比剂注射法判断肺静脉封堵良好的灵敏度、特异度、阳性预测值、阴性预测值、受试者工作特征曲线下面积分别为92.9%、81.0%、96.7%、65.4%、0.882。结论:新型介电传感标测系统对比剂注射法用于CBA中评估肺静脉封堵情况是可行的,且准确性较高。

abstractsObjective:To verify the feasibility and accuracy of a novel dielectric sensing tool, injection module, in assessing pulmonary vein (PV) occlusion during cryoballoon ablation (CBA) of atrial fibrillation (AF) .Methods:Consecutive patients with AF who underwent CBA with the help of KODEX-EPD system in General Hospital of Northern Theater Command from August 2021 to January 2022 were enrolled in the analysis. After a detailed image reconstruction of left atrium and PV was obtained with KODEX-EPD system and Achieve catheter, the degree of CB-PV occlusion was assessed using the "contrast medium injection module" of an occlusion tool (KODEX-EPD) and compared with PV-angiography (the traditional "golden standard" ) .Results:Thirty-five patients with a mean age of (61±8) years were enrolled, 27 males (77.1%, 27/35) , 23 (65.7%, 23/35) with paroxysmal AF and 12 (34.3%, 12/35) with persistent AF. A total of 148 PVs were verified. Optimal PV occlusions were considered to be achieved when no backflow to the left atrium was observed after contrast medium injecting to the PV. The sensitivity, specificity, positive predictive value, negative predictive value and area under curve (AUC) of the "injection module" in identifying optimal PV occlusion were 92.9%, 81.0%, 96.7%, 65.4%, and 0.882 respectively.Conclusion:The "injection module" of dielectric sensing system, KODEX-EPD were feasible and accurate in assessing PV occlusion in CBA.
