PNH异常红细胞的计数及其意义

摘要采用乙酰胆碱酯酶(AchE)单抗结合荧光免疫标记红细胞,通过流式细胞仪直接对单个红细胞膜上AchE进行定量检測,发现阵发性睡眠性血红蛋白尿症(PNH)患者体内红细胞可分为两个类群,一群AchE含量正常[AchE( + )],一群AchE缺失[AchE( — )]; AchE( —)红细胞百分率与蛇毒因子(CoF)诱发的补体溶血率呈正相关;经CoF诱发补体溶血后,仍可发现有对补体不敏感但无AchE的异常红细胞,说明对补体不敏感的红细胞仍有其它异常。本法有可能成为一种比补体溶血试验更灵敏、更特异并易定量的PNH诊断方法。

abstractsBy using indirect immunoflurescence analysis with monoclonal antibody to acetylcholinesterase (AchE) and flowcytometry , the AchE content on erythrocytes from normal donors and 13 paroxysmal nocturnal hemoglobinuria (PNH) patients were assayed . The re sults showed that erythrocytes from normal donors all contained detectable AchE, but erythrocytes from PNH patients could be divided into two distinct populations according to their AchE contents ;the AchE-posi-tive and the AchE-negative . The percentage of AchE-negative erythrocytes was positively correlated with the extent of CoF inducing complement hemolysis . Af ter hemolysis , there were still AchE negative erythro cytes in the remaining intact erythrocytes , indicating that not all complement insensitive erythrocytes were perfectly nomal. The method reported might be a more specific and easier way for the diagnosis of PNH.
